WOODBURY CITY PUBLIC SCHOOLS Preschool and Kindergarten Registration

All registrations for new students will be held at Evergreen Avenue School Appointments are not necessary

Tuesday, March 28, 10:30 am – 1:00 pm Wednesday, March 29, 10:30 am – 1:00 pm Thursday, March 30, 1:00 pm – 3:00 pm Children must be 3, 4, or 5 years old by October 1, 2017 to be eligible for fall.   

3 year old – half day or full day preschool 4 year old – full day preschool 5 year old – full day kindergarten

Preschool Open House Preschool Open House will be held on March 23, 2017 from 9:30 am 10:00 am.  Children who will be 3 years old by 10/1/17 should visit West End School to see a three year old preschool class.  Children who will be 4 years old by 10/1/17 can visit West End School, Evergreen Avenue School, or Walnut Street School to see a full day preschool class. At the time of registration, the following information will be needed:  Original Birth Certificate  Proof of residency (3 forms): o o

Deed (if homeowner) or signed lease or notarized letter from landlord (if renting) 2 additional forms – Examples: Utility bill, telephone bill, driver’s license

 History and Physical form, dated within the last 365 days  Immunization Record: (effective 7-2012) o o o o o o o o

DPT – 4 doses HEP-B – 3 doses Polio – 3 doses MMR – 2 doses Varicella – 1 dose HIB – 1 dose after 1st birthday Pneumococcal vaccine - 1 dose after 1st birthday Influenza vaccine (Preschool only due September through December annually of school year)
