WOODMENLIFE CHAPTER # 302 2017 Fayette County Scholarship Application Mail Completed Application to: WoodmenLife 742 S. Jefferson St La Grange, TX 78945 ALL APPLICATIONS MUST BE POSTMARKED BY MAY 1ST. NO EXCEPTIONS. ELIGIBILITY: Scholarships are available to 2017 graduates in Fayette County who presently hold a WoodmenLife policy. ALL APPLICATIONS MUST INCLUDE THE FOLLOWING: HIGH SCHOOL TRANSCRIPTS: An official high school transcript indicating grades and class rank at least through mid-term of senior year must accompany the scholarship application. LETTER OF RECOMMENDATION: Applicant must submit at least one letter of recommendation. This can be from a teacher, principal, counselor, pastor, or employer. SAT or ACT Scores: An SAT or ACT score is required. WoodmenLife considers the student’s highest one-day test score for scholarship competition regardless of how many times the test has to be taken. PHOTO: A recent photo of the applicant must be included with this application. RECEIPT OF FUNDS: Successful applicants’ funds will be paid to the college/school of higher education of your choice by September 15, 2017, upon receipt by WoodmenLife Chapter #302, of evidence of attendance or acceptance of admission for the 2017 fall semester at an accredited school of higher education. This may include copies of:
Official class schedule Letter of acceptance from the admissions office Receipt for deposit/payment of tuition
We cannot accept class schedules printed from school websites, unless they clearly include the school name, year and student’s name printed from website page. NOTE: Double check your application package. Make sure all items are submitted at the same time. Keep this information and check list. Do not include it with your scholarship package.
