SERVICE FREQUENCY AND COMMUNICATION The facility will be serviced monthly after open hours (unless agreed upon by the RGM and Service Specialist). The Restaurant General Manager or Area Coach is to provide keys/codes for restaurant access. A printed service report is provided after each visit and filed in the log book. Service information will be recorded using the PestService App, an electronic documentation system that is used to generate on-site, easy-to-read reports. These reports must include any structural and sanitation issues that are relevant to pest issues and details surrounding those issues must be documented in detail. CUSTOMER SERVICE LOGBOOK A Customer Service Logbook will be provided and maintained by Ecolab. This logbook will serve as a reference tool containing a service history of the facility and identification of important safety information. It will include documentation of state licensing and certification, copies of service reports for past 6 months, product labels, Safety Data Sheets (SDS) and copy of facility diagram with trapping device locations noted. TARGETED PESTS Targeted pests will include cockroaches, rats, mice and ants (excluding pharaoh ants, fire ants, carpenter ants, or any other wood destroying insects). In addition, Ecolab will treat for occasional invading pests such as crickets and other crawling insects during regular service visits. Additional services that require an extra trip will be charged an additional $157.50. Service Specialist will communicate additional cost with Restaurant General Manager prior to any additional services being performed. SERVICE RESPONSE TIME Ecolab will respond verbally within one hour. (If SVSP is not available due to paging times District Manager or Associate District Manager will respond to RGM) For confirmed Emergency Issues, Service Specialist will be physically on site within 4 hours after management calls to report a pest that is guaranteed under the Pest Elimination Agreement. All other service response will be within 24 hours or agreed upon time. In addition, Ecolab will provide emergency service as long as this does not interfere with the normal operations of the facility. ESCALATION POLICY Yum! Brands have a very specific escalation policy as outlined in the flow charts attached. Some highlights of the program are: 

Daily service will begin with an infestation. In the event of a rodent infestation YUM! will consider a voluntary closure of the unit.

Action plan and detailed information for issue resolution should be provided to the Area Coach at the beginning of the escalation of service.



Notify Facility Leader after service returns to monthly to communicate issue resolution.

SANITATION AND STRUCTURAL INSPECTION A sanitation and structural inspection will be conducted on each service visit and findings will be included in the service report. All service areas listed in the “Service Frequency and Scope” section of this document will be inspected. Inspection details will include any pest activity found (including type of pest), along with structural and/or sanitation conditions that may contribute to pest activity or infestation. It is critical that a detailed description of the issues be documented. Ecolab will make specific recommendations for structural and sanitation improvements and work closely with management to encourage implementation of these improvements. WEB REPORTING AND E-ALERT Ecolab will provide web reporting and e-alert for all corporate YUM locations. The e-alerts will be channeled to the Quality Assurance Department of YUM. If during a service visit live cockroaches or a live rodent is found, it is to be documented and an E Alert will be sent to YUM corporate for further follow up with the RGM. Sanitation and structural issues will also be sent via E Alert for repetitive issues that have not been repaired/remedied since identification at last service visit. ECOLAB RODENT PROGRAM™ All rodent equipment placed at the facility will be documented on a facility diagram that will be updated whenever changes are made. The facility diagram will be kept in the service logbook. If equipment is lost or thrown away, a replacement charge of $10.50 for multi catch trap and checkpoint traps will be charged. Exterior: To reduce rodent pressure on the exterior of the facility, Ecolab will install and maintain three (3) exterior rodent bait stations. One at the dumpster area and two on the exterior of the building out of public view, generally at the rear service entry door. Ecolab proprietary bait stations will be used and will include the Check Point Rock station or the VENT Station. All bait stations will be secured to the ground locked and contains date/product cards. Ecolab will service these bait stations monthly. These stations are not to be placed in public view. If discreet location is not available, use vent style device. Document on the rodent schematic located in the log book “no bait station near the dumpster due to lack of or missing fencing” Interior: Ecolab will place and maintain interior, multi-catch rodent traps. Ecolab will service these traps monthly. In addition to these traps, glue boards, snap traps and other traps may be used as needed to eliminate interior rodent infestations. All traps will be placed discreetly to minimize impact on normal facility operation. No snap traps are to be used on the floor level unless restaurant is closed for an infestation treatment.

PERIMETER PROTECTION PROGRAM To protect against crawling insect pests, Ecolab will apply residual products to exterior doorways and other entry points of occasional invaders. These applications will be made during regular service visits and will typically be done using a compressed-air sprayer. Additional service options such as power spray may be recommended if exterior pest pressure exceeds these protective measures. The additional cost will be $157.50. ECOLAB COCKROACH PROGRAM™ The Cockroach Program is a proven integrated pest elimination approach that uses multiple strategies to first eliminate, and then protect your facility from cockroach infestation. Along with this program, five (5) insect monitors are to be strategically placed throughout the restaurant not in public view.

Optional Programs described below require Facility Leader approval. LARGE FLY PROGRAM™ - OPTIONAL (Pricing should be obtained through CAM/CAS) The Large Fly Program consists of multiple strategies on both the exterior and interior of the facility to minimize house fly and other large fly activity. Ecolab will install and maintain this program on an asneeded basis in the months each year while flies are active. Service of this program will begin at the beginning of fly season and will continue through the end of fly season. Some areas will consist of a six-month fly season and others will consist of an eight or twelve-month fly season. Air Defense NOT ALLOWED. Exterior Ecolab will apply fast-acting, long-lasting residual products to fly resting surfaces such as garbage storage areas and building entryways and can include fly bait station. Please refer to the specifics in the YUM! Brands bid specs. Interior Ecolab will strategically place and service Stealth Fly Traps on a monthly basis. This service will include the replacement of Stealth glue boards each active month of the program and bulbs as needed. Residual products will be applied to interior fly resting surfaces. Fly Bait Station

ECOLAB SMALL FLY PROGRAM™ - OPTIONAL $325.00 per Treatment The SMALL FLY Service targets fruit flies and drain flies in your facility using a proprietary 3-cycle approach. 1. Sanitize - renders fly breeding sites unsuitable for egg laying. 2. Wash - removes larval food sources. 3. Treat - knocks down existing adult fruit fly populations and provides lasting protection. This service will include the use of Small fly Treatment. Small Fly Ovicidal Treatment, a proprietary product used only by Ecolab. Do not leave chemicals on site for Yum employees to use. ECOLAB AIR QUALITY PROGRAM™ - OPTIONAL $11.55 Per unit The ECOLAB Air Quality Program eliminates odor issues in your facility. Ecolab will install and maintain Air Defense odor units on each service visit. A variety of scents and odor counteractants are available. TERMITE PROGRAM - OPTIONAL Price based on Survey Ecolab will conduct a thorough inspection of the facility for evidence of termite activity and/or conditions conducive for termite infestation. If warranted, Ecolab will provide integrated termite service options that may include the use of termite monitors, baits, and/or the use of liquid termiticides. In the event that termite activity is found during an inspection, a separate scope of service will be provided for termite elimination. (Additional fees will apply.) Ecolab uses an integrated approach to protecting facilities against subterranean termites. Treatment methods will vary depending upon the intensity of termite activity present inside and outside the structure. Treatment options include: Spot Treatments: Spot treatments are used when termite activity confined to small area within the structure. These treatments may include the use of liquid termiticides and above ground bait stations. Barrier Treatment: Barrier treatments are provided when termite activity is found in many locations within the structure. This treatment consists of establishing or re-establishing a complete barrier around the entire facility. A treatment warranty of up to five years will be available provided annual inspections are performed and the integrity of barrier is verified. Monitor/Baiting: A monitor/baiting service will be used when there is termite activity outside of structure. This option provides a perimeter system to reduce activity near the structure and minimize future termite attacks on the structure.

Ecolab associates will evaluate the termite situation of each facility and make recommendations to Facility Leader as to which method or combination of methods will provide the best structural protection.

WINGCOMMAND BIRD PROGRAM™ - OPTIONAL The Wing Command Bird Program provides solution options for bird pest issues. Nesting, roosting, loafing and feeding birds will be excluded and/or repelled through the use of multiple service options. Some of the options available include: Electric Track:

a low-amperage deterrent that deters birds from roosting and loafing on ledges.

Coils:

a wire system that prevents birds from landing by creating an unstable landing platform

Rotating Wires:

a device to keep birds off streetlights and other elevated perches.

Spikes and Wires:

nearly invisible options to protect all landing and ledge areas.

Stainless-Steel Rods:

an embedded, durable base to defend against Pigeons and Gulls.

Netting:

an exclusion device installed to block entry of birds into sensitive areas.

Repellent Barriers:

Non-poison application to active areas.

A separate scope of service will be provided for Wing Command Bird Program services. (An additional fee will apply.) INSURANCE AND REGULATORY COMPLIANCE A specimen copy of Ecolab liability insurance coverage, examples of an SDS and product labeling are included with this scope or service. Your customer logbook will contain printed material according to all relevant federal, state and local regulations.
