2015 SHOREMEN BASKETBALL CAMPS For boys entering grades 3 – 9 Grades 3 & 4 Monday, June 8th thru Thursday, June 11th at Avon Lake High School Time 9:00 – 10:30 Grades 5 & 6 Monday, June 8th thru Thursday, June 11th at Avon Lake High School Time 10:30 – 12:00 Grades 7, 8 & 9 Monday, June 8th thru Thursday, June 11th at Avon Lake High School Time 1:00 – 3:00 Each camper will receive a T-shirt and a Shoremen basketball. (In order to ensure that each camper receives a T-shirt, all Registration Forms must be received by May 29th.) I have no knowledge of any physical impairment that would be affected by the named student’s participation in the program. I further consent authorizing emergency medical treatment.
