Dear Student Athlete, Our Sports Medicine Staff would like to welcome you back to Abilene Christian University for another great year in ACU Sports! In preparation for our 2016-2017 sports seasons, there are several items that must be updated by each Student Athlete in order to be medically cleared for participation in your respective sports. Please carefully read and complete all items at your earliest convenience. You may use a cell phone, tablet, or computer to complete the ATS Athlete Portal, as directed below. -Open your browser, and go to https://atsusers.com and follow the ATS Athlete Web Portal Instructions included in this document -All information regarding medical history and health insurance must be updated and entered accurately. Please provide us with accurate, up-to-date information. -If you do NOT currently have health insurance, e-mail [email protected] ASAP so we can assist you in signing up for school insurance prior to physicals. If you were insured through the school insurance last year and will also need coverage for this year, send an e-mail to [email protected] so that we can get your plan renewed for another year. - A returner’s sports physical will be completed with the Athletic Training staff when your team reports to campus. It is not necessary for you to get a physical completed by a physician for this. We will update these when you come to campus. -If you have received physical therapy, or have been seen by your own physician at home for any condition regarding your health/well-being, be sure to provide us with all documentation about your care/services. - If you are currently being treated for ADHD with a stimulant medication, this MUST be reported using the NCAA Medical Exception Documentation Reporting Form to Support the Diagnosis of Attention Deficit Hyperactivity Disorder (ADHD) and Treatment with Banned Stimulant Medication -Carefully read the instructions attached to this e-mail to make this an easy, problem-free process If you have questions regarding this system or any questions regarding insurance, please contact Adam Ybarra at [email protected]. ABILENE  CHRISTIAN  UNIVERSITY  SPORTS  MEDICINE   RETURNING  ATHLETE  WEB  PORTAL  INSTRUCTIONS       Website  Information     https://www.atsusers.com/   Click  on  Athlete  Portal  in  the  middle  of  page     Logging  into  the  ATS  System:   Users  with  accounts:   Athlete  ID:  the  ID  you  created  last  year  and  should  be  your  first  initial  and  last  name  (example:  Ron  Smith,  would  be   “rsmith”)   Password:  this  password  was  created  by  you  last  year.  If  for  some  reason  you  cannot  remember  your  password  click   “forgot  password”  or  e-­‐mail  [email protected]  so  we  can  send  it  out  to  you.   Database:  atsacu     Athlete  Information  will  immediately  open  on  your  screen,  with  the  “general”  tab  open.  All  of  this  information  must  be   reviewed  and  updated  as  necessary.  It  is  important  to  open  EACH  tab  and  follow  the  directions  below!!     Items  that  may  need  updated:   § -­‐Cell  phone  #  and  text  message  #   § -­‐E-­‐mail  (update  to  your  ACU  school  e-­‐mail  if  this  has  not  already  been  done)   § -­‐Address  if  it  has  changed     § -­‐Year  in  school  (must  be  changed)   § -­‐Medical  Alerts   § -­‐Allergies   § -­‐Current  Medications-­‐any  medications  that  have  been  added  to  your  daily  routine  must  be  included   Save  and  Verify  Athlete  General  Information     Medical  History:     All  major  injuries,  illnesses,  and  surgeries  must  be  entered  here  under  Medical  History.  Events  that  occurred  in   the  last  year  must  also  be  accurately  updated  into  this  Medical  History  section  (surgeries,  season-­‐ending   injuries/illnesses,  concussions,  etc).                 To  add  new  surgeries:                                                                                     § Click  “+Add”     § Complete  the  pop-­‐up  box  and                then  click  the  “insert”  check-­‐box   § Type  in  any  other  medical  notes,              regarding  both  sport-­‐related  and              non-­‐sport  related,  injuries/illnesses              in  the  “Other  Medical  Notes”  box                    Save  and  Verify  Medical  History                  

    Immunizations  &  Paperwork:     Make  any  changes  to  immunizations   that  have  been  received  in  the  last  year  (example:  tetanus,  etc.)    

  Verify  Immunization  Information  if  changes  were  made     Insurance:   § Click  on  the  Insurance  tab       § Review  the  Insurance  details     and  complete  any  new  required  fields   o MUST  enter  the  policy  holder’s                  DOB  and  SS#,  Insurance  group  #                and  policy  #   § If  your  insurance  has  changed  from     Last  year,  push  the  “delete”  button   at  the  top,  and  then  click  “+add”  to     add  your  new  insurance.     o If  new  insurance  was  added,     Insert  a  FRONT  AND  BACK                picture  of  your  HEALTH  insurance                  card.  If  this  is  NOT  an  EASY  TO                  READ,  CLEAR  picture,  then  DO                NOT  ADD  IT.  If  the  picture  is  hard                  to  read,  bring  your  INSURANCE                  CARD  to  physicals  so  we  can  scan                it  in  with  our  scanner,  or  e-­‐mail  it     to  [email protected].  This  MUST  be  completed,  OR,  you  must  have  your  actual  insurance  card  at  the  time   of  physicals   § If  no  changes  were  made,  click    Verify  Insurance  Information   § If  new  insurance  was  added,     be  sure  to  Save  Insurance  Information     All  ACU  Student-­‐Athletes  MUST  have  primary  insurance!  If  you  do  not  have  insurance  contact  Jenny   Scruggs,  Insurance  Coordinator,  ([email protected])  so  we  can  sign  you  up  for  school  insurance   prior  to  physicals.     If  you  are  unable  to  upload  the  picture  of  your  insurance  card  to  ATS,  you  can  e-­‐mail  it  to   [email protected].       Emergency  Contact   § Click  on  “Contacts”  tab,  and   enter  Emergency  Contact   information   o Enter  2  Emergency   Contacts   o Be  sure  that  the  contact   information  is  current   and  correct   o To  add  a  new  contact,  or   edit  a  current  one,  use   the  “add,”  “edit,”  and   “delete”  options  at  the   top   § If  changes  were  made,  click  Save  and  Verify  Medical  History  

ATHLETE  FORMS   § Click  on  “Forms”  at  the  top                                                         § There  are  several  forms  that  must     be  completed  EACH  year   § Under  “Form  Name”  each  of  the  following   forms  need  to  be  completed:     -­‐Athletic  Participation  Statements     -­‐Drug  Testing     -­‐Illness-­‐Injury  Reporting     -­‐Medical  Policy     -­‐Student  Athlete  HIPPA  Auth.     -­‐ADHD  Reporting  Form   § For  each  form,  click  the  form  name,  then     click  “new.”  Read  the  document,  sign  the     document  in  the  box  then  type  in  your     name  in  the  “signed  by”  and  click  “sign,”     then  click  “save”  at  the  bottom   **You  must  save  after  each  one**     § You  WILL  NOT  be  cleared  by  Athletic     Training  staff  to  participate  until  ALL     forms  have  been  completed  

       

If  you  do  not  upload  a  picture  of  your  insurance  card  to  the  portal,  be  sure  to  bring  your  actual  insurance  card  with  you   to  the  physical  exams.  If  necessary,  you  may  bring  a  clear,  front  and  back  copy  of  your  insurance  card  with  you  to   physicals.  These  must  be  verified  in  person  with  the  Athletic  Training  staff  in  order  for  you  to  be  cleared  for   participation.     A  returner’s  sports  physical  will  be  completed  with  the  Athletic  Training  staff  when  your  team  reports  to  campus.  It  is   not  necessary  for  you  to  get  a  physical  completed  by  a  physician  for  this.  We  will  update  these  when  you  come  to   campus.     If  you  have  received  physical  therapy,  or  have  been  seen  by  your  own  physician  at  home  for  any  condition  regarding   your  health/well-­‐being,  be  sure  to  provide  us  with  all  documentation  about  your  care/services.         If  you  have  any  trouble  with  the  above  process  please  call  Adam  Ybarra,  Head  Athletic  Trainer,  at  325-­‐674-­‐2230  or   email:  [email protected]     Sincerely,   Sports  Medicine  Staff
