Contact info: 5013586583 during working hours or 5017333809 This program is offered by St. Joseph Catholic School as a service to the families of St. Joseph School for PreK4 & PreK3. Our goal is to provide a quality extended program in a safe, secure environment, and support working families.
All Registration Forms must be completed in order for a student to attend EP Extended Program tuition/rates are as follows: Registration Fee: $25.00 Is there a sibling that attends the ASP? YES/NO or will be Also attending Extended Care YES/NO. Please be sure to let us know. Plan A 5 days per week $120.00 / 9 months Plan B 3 days per week: $96 per month/ 9 months Plan C Dropins 1 day of service: $10 st st st th All fees/balances are due at the 1 of each month beginning September 1 ending May 1 and become delinquent on the 10 of the month. If an account becomes delinquent a $15.00 late fee will be charged and the student/s will not be allowed to attend EP until the fee has been paid in full. Make all checks payable to St. Joseph Catholic School Extended Program. A change in payment plan can only be made at semeste r unless other circumstances are discussed with the director. After School Program will end daily at 5:30 p.m. A late charge of $1.00 per minute will be charged for each child picked up after this time and due at the time of pickup. * Because family matters discount If you have another sibling attending but at the ASP location. We will waive your registration fee and discount you 50% off ONLY if you are on either Plan A or B at BOTH locations. If it is expected that the program will be used only on an occasional basis, please fill out all paperwork at the beginning of the school year. Then the records will be complete when the student attends E.P. Students are to follow the rules listed under the “Discipline Policy”.

Discipline and Tuition/Rates Policies Agreement I understand and have read to my student/s the rules for behavior expected at St. Joseph Extended Program. Additionally I have read and understand the policies included in the Program tuition/rates information. I understand all payments are st due at the first of each month starting in Sept 1 and ending May1st. I am only to change the plan at the beginning of the month. I understand that I may ask for a conference with the Director and/or Assistant Director as needed.

No inappropriate physical contact including hitting, biting, kicking, wrestling or spitting. No rough housing or excessive noise in the building. No pets. No foul language. No bullying including name calling or putting people down. Always treat each other with respect. Playground rules: No picking up or touching rocks, sticks or other dangerous objects. Always share and let everyone play. 7. Snack time: Be courteous to others and use appropriate manners. 8. No trading/or selling of cards or video games. 9. Only “E” rated games are allowed at E.P. 10. No inappropriate pictures or videos with electronic devices to be taken. 11. Cell phones or computer/smart phones etc. are to be used for homework, games, and parental contact only, unless approved by the director. 12. Director reserves the right to look at your electronic device at any time during EP to ensure proper use and no bullying of other students. 13. If a student breaks a rule, he/she will sit quietly to allow them time to think about the rule and come to a decision regarding future behavior. This time is not considered punishment but “time out”. If a student continues to willfully break the rules or cause harm to another student, the parents will be notified, a meeting will be scheduled and the child will be placed on probation for one week. If the behavior continues to be a problem, the child will be dismissed from the Program. **It is the responsibility of the parent/guardian and the child’s teacher to send the student to E.P . Please inform your child’s teacher or notify the school office specifying if your child is or is not to attend E.P especially if it is a change from the regular routine. Please call the office before 3:00 if there is a change of plans. The office will then inform the teacher and student of the change.
