2017 Senior Scholarship Application
2017 FIHA Peter Courcy Senior Scholarship
Frisco Ice Hockey Association is proud to announce we will be offering a scholarship for any of our 2017 seniors who wish to further their education. Any JV or Varsity player may apply for our scholarship. The scholarship award will be based on the following:
Participation in FIHA Character of participant Current school transcript and report card Community service activities Future academic plans
Scholarships will be funded solely by excessive revenue in the FIHA base account and/or from funds received by donations. Players’ fees will not be affected and will not increase due to the inclusion of this scholarship in our program. The FIHA will work with the Frisco Education Foundation to evaluate each application and present a recommendation to the Board of Directors for final approval. Applications will not be judged on others who apply. The application must be turned in no later than March 2, 2017 to Bryan Sinclair, FIHA Treasurer, or may be mailed (must postmarked by March 2, 2017) to the FIHA address listed below. If you are going to be mailing your application, send Bryan an email at [email protected]. Upon receiving your scholarship packet, Bryan will send you a confirmation of receipt. Please be sure to make copies of what you send in. Late applications will not be accepted. Incomplete applications will not be considered. All applicants must be in good standing with USA Hockey and Frisco Ice Hockey Association. You may submit to Bryan Sinclair or mail to the FIHA address below. Please note that we appreciate neatness and creativity. Character and personal references (3 total required) o From present year sports coach, athletic director, or team manager (any sport) o From a current/past teacher or counselor or school administrator o From a friend or community member o From a current or past employer Transcript of grades o Copy of most current issued transcript of grades o Copy of most current report card
Honors , awards, school organizations/clubs o List any scholastic and sports awards you have received o List any school organizations or clubs you are in
Essay o Must be no longer than two pages (double spaced) “What playing hockey for Frisco Ice Hockey has meant to me”
Community service o List any non-FIHA community service that you have done over the course of your High School career and provide authentication Participation in FIHA o Years in the program o Raffle sales o FIHA Community Service Projects Participation
Future educational plans o Tell us your plans for furthering your education
