APPLICATION FOR ADMISSION 2018 VAN COMPTON VOLLEYBALL CAMPS

CAMPUS POLICY

Campers participating in the Van Compton Volleyball Camps must remain on the University campus unless the camper checks out of camp with a parent, legal guardian, coach, or staff member. If a camper fails to follow this policy, her parents/legal guardian must assume all liability responsibility.

The Jack Stephens Center FACILITY AND ELIGIBILITY STATEMENT

Most camp sessions are held at the Jack Stephens Center, a 5,600-seat facility that is the site for all Little Rock Trojans volleyball games. All camp sessions are open to the public and parents/friends are encouraged to attend. Team camp sessions will also be held at the Donaghey Student Center and at various gyms on campus. The camp is open to any and all entrants limited only by number, age, grade level and/or gender.

PHYSICALS

All campers must send in this application or bring to registration a 2017-18 physical signed by a doctor. Campers must have this to participate in camp.

MEDICAL INSURANCE

Each camper is covered by a secondary $25.00 deductible excess medical insurance policy. A secondary excess policy simply means that the camper’s insurance policy is the primary coverage and that the excess camp policy will pay what the primary policy will not. The insurance policy is an accidental policy and will not cover illness. Only injuries occurring while the camper is enrolled in camp will be covered. Injuries received prior to the date of camp and injuries occurring after the camp has ended will not be covered under this policy. The deductible must be paid before the camper checks out of camp if there is an injury report filed.

WHERE TO REPORT/REGISTRATION/AWARDS • Position Camps • Registration is at 5 p.m. at The Jack Stephens Center on the main level on the first day of camp. All grades register at this time. Balance of fees will be due at registration. Hitters and Setters Camp is July 9-10 while Serve/Serve-Receive Camp is July 11-12. Defensive Specialist Camp is July 13.

• Individual Camp • Registration is at 5 p.m. at The Jack Stephens Center on the main level on July 16. All grades register at this time. Balance of fees will be due at registration. • Team Camps • Registration is at 7 a.m. at The Jack Stephens Center on the main level on July 23 for senior high and July 25 for junior high. Balance of fees will be due at registration. REFUND POLICY

To guarantee a space in camp, the camper must either enclose the full payment or a deposit equal to half the cost of camp. The preregistration deposit is non-refundable. In case a camper pays the entire amount prior to the start of camp and cannot attend, she may request and receive a refund for everything but the cost of the preregistration deposit (half of the full amount). This refund request must be in writing and must be received by the camp director prior to the start of camp.
