Dear Parents/ Carers We would like to inform you of some fantastic sports clubs at All Saints Academy. Full list of clubs below; How to Book: Complete the attached Booking Form with payment by either cash or by cheque. Cheques must be made payable to Sean Hughes. Please return the forms to the school reception in a sealed envelope with your child’s name and ‘Sport2Day’ written on the front as soon as possible as places on the courses are limited. I give permission for my child to walk home alone: I certify that the child I am enrolling is aged 4 years old or over and attends school full time. I fully understand that if the child is not 4 years old and attending this school full time, or if their behaviour is unacceptable to Sport2day, they will be removed from the School Club and no refund will be given. In the event of an accident I hereby give my consent for a trained member of staff to adminis‐ ter first aid or to seek emergency medical advice or treatment on my behalf.

In the event that Sport2day has to cancel and event for any reason a full refund will be issued to the person who booked, no written request is required. Sport2day may from time to time take photographs/film of clubs; the use of this data is solely for the purpose of promoting and publicising Sport2Day events . Parents/ carers that do not wish for their child to be photographed/ filmed must make Sport2day aware of this at the time of booking or in writing before the activity starts.
