Dear Parents and Guardians,

I am pleased to announce that we will be moving to an electronic process. If you have not yet logged on to view the new Athletic Website, please do so by going to the "Athletic Tab" on the High School website or by going directly to winslowathletics.com. Registration will be available on the website for most teams starting July 9th 2016. Please note that ALL athletes are to register, even if they turned in a physical packet at the end of the school year. No hard copies of the participation forms will be accepted. To begin, click on the “Registration” tab to the right of the screen to get started. Here you can sign most participation forms electronically which includes the (Permission Slip, Code of
Conduct, Photo Consent, Cardiac, Impact, Concussion, and Steroid). Physical forms can also be downloaded and printed during registration. Forms that are considered “Physical Forms” include (Physical Packet, Health History Update, Medication Admin, and Asthma Action Plan). Physical forms MUST be printed out with the hard copy of the original physical completed and submitted to the Athletic Department. If the medication administration or the asthma action plan form applies to your child these form MUST be stapled to the physical packet and submitted. We have added an additional “School Physical Day” of July 21st at 1pm. If you are getting a physical outside of school, physicals should be turned in before July 20th in order to avoid a delay in being able to practice. The official start of the season starts August 10th 2016 and all students must be completely cleared by this
date. Please remember a completely cleared student has all participation forms electronically submitted, specialty clearance forms submitted, and physical form submitted with a passing review by the Winslow Township School District’s Physician. The website is a work in progress and we will continue to refine and update it as we move closer to the 2016/2017 school year. ARCHIEVED LETTER APRIL 2016: Athletic Physicals 2016-2017 School Year Dear Parents and Guardians, This letter is to inform you that there have been changes to Winslow Twp. High School’s Athletic Participation Physical procedure for the 20162017 school year. It is important that your son or daughter get a new physical if their physical expires before March 10, 2017. We recognize that there have been some concerns in regards to students retaining a valid NJ Sports Physical at their family doctor. Affective March 2015, New Jersey law now requires each doctor to hold a Cardiac Module Certificate. Please be aware that if your family doctor does not hold this certification then any physical that they give will not be valid for participation in a school sports. Before getting
a sport physical at your family doctor please inquire whether the doctor has completed the cardiac module. Previously, we have offered physicals during the months of September, October, March, and May. Effective May 2016 we will only be offering physicals during the months of May (May 24, 2016) with a make-up date of June 1, 2016. An additional physical day of July 21, 2016 at 1pm has been added. Physicals in July will serve as a make-up day and a freshman physical day. Moving forward we will attempt to adhere to this outline each year for the summer and fall athletic seasons. A freshman make-up day, if any, will be announced on the Athletic Website. Registration will be conducted through the Athletic Website where parents can find all of the required forms the month prior to physicals. On the site pre-participation paperwork can be
signed electronically. Once all necessary forms are signed and submitted, the student will be considered “registered.” On physical day the districts medical staff will conduct physicals for up to 100 student athletes on each day. Students are encouraged to stand in line when the bell rings to ensure they are amongst the first 100 students. If a student cannot participate in physical day they reserve the right to obtain a physical from their primary, which must be submitted before May 24, 2016 and approved by the school physician. For the newly added physical date of July 21st registration will open on or around July 7th and will close on August 10th be sure to register via the school’s athletic website. This new schedule will ensure that your child's physical, once received, is good for the entire academic year, therefore, for all four seasons! IMPORTANT THINGS TO KNOW:
Physical must be on NJ State forms. (No other state’s form will be accepted) Transfer students will need a physical and participation forms Make sure that your primary doctor fills out the physical in its entirety. Ex. (Eye Exam, Hernia Check, All signatures and date. Be sure to attach any supporting forms. (Attach Asthma Action Plan, Clearance note from any specialist treated by. (Cardiologist, Orthopedic, Pulmonologist ec.) *Without these support forms the physical will be marked a “fail” and may delay your child from participation and or tryouts.
