AYSO Region 65 Flames Memorial Fund Scholarship Program
AYSO Region 65 Flames Memorial Fund Scholarship Program College Scholarship Application Region 65 seeks to honor one local man and woman who are graduating seniors with scholarships in the amount of $1,000.00 each. Applicants must meet the following criteria:
Is a graduating High School Senior from one of the cities served by AYSO Region 65. Have a 3.0 or better GPA. Have played AYSO soccer for a minimum of 3 years, 1 of which must be in Region 65. Have served in a volunteer capacity to AYSO. Submitted a complete application, along with supporting documents prior to the deadline.

Letter of reference from an adult affiliated with AYSO Region 65 who is familiar with your AYSO career as a player and / or volunteer. (May not be from a relative). 3. Verification of your GPA. This may be in the form of a report card, school transcript or signed letter from a school official on school letterhead. Staple all documents in the order listed, using the application as a cover sheet. Seal them in an envelope and mail to: AYSO Region 65-Scholarship Committee P.O. Box 9208 Rancho Cucamonga, Ca. 91701 All entries must be received by June 15th in order to be eligible for consideration.
