AYSO Region 677 Leah Martindale Memorial College Scholarship Leah Martindale was a devoted referee and board member, giving many hours of her time to the Region. She passed away in January 2012 at the age of 37. This scholarship is awarded to honor her memory. AYSO Region 677, Canyon Country will award two (2) scholarships in the amount of $1,500, to be used for higher education, to AYSO volunteers who have demonstrated a commitment to volunteer services and academics. Interested students who meet the criteria must submit a completed application and Essay by April 1. Applicant Criteria: 1. Must be a graduating high school senior. 2. Must have been a registered volunteer during the current AYSO Region 677 Fall season. 3. Must have supported AYSO Region 677 through volunteer activities, for a minimum of two (2) seasons. 4. Must have a cumulative High School GPA of 2.5 or better, verified by official transcripts. 5. Must plan to pursue higher education at an Accredited Community College or 4-year University or an Accredited Technical/Vocational School. Applicant Checklist: 1. Complete all sections of the Application. 2. Include the following documents (copies are acceptable) • Application form • Essay - How has your involvement in AYSO influenced or enhanced your life? Essay must be between 300-500 words, double-spaced and well written. • Most recent transcript available. 3.
