I give my consent for him/her to participate in the camp programs conducted and/or sponsored by the Bonnie Henrickson Basketball Camp. I understand that participation in basketball, and related activities, involve certain risks and may result in unavoidable injuries. The injuries may include muscle strains and tears, broken bones, and severe injuries including, but not limited to, permanent paralysis, or even death. I am fully aware of the risks and possibilities of injury involved and acknowledge that I am assuming the risk of such injury by my child participating in the camp. • I further acknowledge that I will be responsible for any and all medical and related bills that may be incurred for any illness or injury that my child may sustain during the camp and while traveling to and from the site of the camp. Employees of the Bonnie Henrickson Basketball Camp, Kansas Athletics, Inc., or the University of Kansas to act accordingly in any situation requiring medical attention, whether an emergency or not, until such time as I am contacted to make decisions concerning my child’s treatment. If in the judgment of a physician or designee it is necessary for health care reasons to proceed with treatment without delay, this treatment may proceed without prior notification of the undersigned, although every attempt will be made to notify me in the event of such an injury or illness. I agree that any medical information provided to this camp shall be released to other health care providers who may be providing care.• Knowing these facts, and in consideration of my child’s participation in the camp program, I, acting as parent or legal guardian, agree to release and hold harmless the respective officers, directors, representatives, members, agents, employees, coaches, or agents of the University of Kansas, the Kansas Board of Regents, Kansas Athletics, Inc., the University of Kansas Recreation Department, the State of Kansas, the coaches and support staff of the Kansas women’s basketball program, from any and all liability for negligence or any other claim, demand, action, judgment, loss, liability, cost and expenses (including limitations, attorney’s fees and costs) arising out of or in connection with the camp, including any claim arising out of or in connection with, whether directly or indirectly, any illness, injury, damage or loss to person or property that my child may incur or sustain during the camp, all activities associated with the camp, and while traveling to and from the site for the camp. • I further acknowledge and authorize the Bonnie Henrickson Basketball Camp the right to photograph my child and use the photo and/or other digital reproduction of him/her or other reproduction of his/her physical likeness for publication processes whether electronic, print, digital or electronic publishing via the Internet. • I acknowledge that I have read this Release and Participant Waiver of Liability in its entirety and fully understand its contents. I am aware that this release contains an acknowledgement of my voluntary and knowing assumption of the risk of illness or injury. ELITE CAMP JUNE 5-7

The 2014 Bonnie Henrickson Basketball Camps will be held at The University of Kansas and will include Kansas Athletics facilities as well as other on-campus gymnasiums. Head Coach Bonnie Henrickson and her staff will oversee all camp sessions. Current and former KU players will also assist with instruction and supervision. All overnight campers will reside in Naismith Hall, located on the corner of Naismith Drive and 19th Street in Lawrence. Meals will be served in the dining facility of Naismith Hall. All facilities are within walking distance of one another. Elite and Individual Camps require a $100 deposit to hold reservation for camp. Final payments are due by dates listed below to avoid cancellation of reservation. Elite Camp - Thursday, May 15, 2014; Individual Camp - Friday, May 30, 2014. All accounts must be paid prior to the start of camp. Full payment is required for registration to the Junior Jayhawk Camp. All camps are open to any and all players and will only be limited by age/grade, gender and number of participants. CANCELLATION/REFUND POLICY

No refunds will be issued for the Jayhawk Jamboree or Junior Jayhawk Camp. Refunds for Elite Camp or Individual Camp will only be processed in the event of illness or injury PRIOR to the start of the camp session. All requests for a refund must be received in writing PRIOR to the start of the respective camp session. Requests must include medical documentation outlining the limits to participation as well as the signature of a licensed physician (unrelated to the camper). The $100 deposit and any on-line registration fees will be deducted from the refund amount. Registrations are non-transferable from year to year. No partial refunds will be given to any request made after camp begins or for any camper sent home for disciplinary reasons or due to homesickness. All returned checks are subject to $50 charge. Elite Camp is designated for the advanced player wanting to improve individual/team skills during the off-season. Players will be introduced to Coach Henrickson’s system and philosophies of the game. Emphasis will be placed on developing advanced skills according to position both offensively and defensively. All members of the KU coaching staff will be present at camp in addition to current and former KU players, high school coaches, professional players and college coaches from other universities. It is strongly recommended that participants have completed at least the 10th grade or have one year of varsity playing experience prior to enrolling in camp.

INDIVIDUAL CAMP JUNE 22-25

Individual Camp emphasizes the development of fundamental skills. Daily instruction and station work are designed to encourage the mastery of basic skills. In addition, players are grouped according to age and skill level. Campers will compete on teams in order to apply their individual skills to team concepts. All members of the KU coaching staff will be present at camp in addition to current and former KU players, high school coaches, professional players and college coaches from other universities. Girls entering grades 5-12 in the fall of 2014 are eligible for Individual Camp.

JUNIOR JAYHAWK CAMP

JUNE 30-JULY 3 (9AM-NOON) Junior Jayhawk Camp is designed to teach fundamental skills and rules of the game to young, aspiring players. Camp will include basic skill stations, 5 on 5 competitions and more. Camp will run daily from 9 am – Noon. This camp is for girls and boys entering grades 1-5 in the fall of 2014.

A one-day team camp that provides a professional atmosphere in which teams can compete against quality opponents. Four game guarantee (8 player minimum per team). Teams will be divided into competition pools (based on head coach recommendation) to ensure quality opponents in round robin format. In addition, all teams will be instructed on NCAA compliance and the college recruiting process. High school varsity, junior varsity and AAU teams are eligible for the 2014 Jayhawk Jamboree.
