CHILDCARE VOUCHER INSTRUCTIONS We are offering childcare vouchers at a fixed rate during Live and Learn. Here is what you do each week: 1. Secure your own in-home childcare. 2. Attend a Live and Learn class and sign the attendance sheet. 3. Fill out a childcare voucher request form:  https://thecreek.formstack.com/forms/childcare_voucher_for_live_and_learn 4. After we receive your request form and match it with the attendance sheet, we will send a voucher to you in the mail up to $25.00 to help cover your childcare costs.

Our hope is that childcare vouchers will help parents, guardians, and families be able to attend Live and Learn.
