College ID Camp January 23rd 2016 Schedule:

The following must be completed by a parent/guardian Cost: $60 (Includes Lunch) Please send payment to: Women’s Soccer, University of Alabama Huntsville, 205 Spragins Hall, Huntsville, AL 35899 Each participant must provide proof of a medical physical, which should be signed by a physician and dated within the past six months. A signed sickle cell release form must also be presented. If the participant is under 18, they must have the waiver and sickle cell release cosigned by a parent or guardian. In the event my daughter is injured or becomes ill while attending the UAH 2016 ID camp, I give my permission for the staff to seek medical attention if deemed necessary under the existing conditions. I release the University of Alabama Huntsville, the coaching staff and athletic trainers from any claims from injuries sustained during the camp. I also certify that my daughter is in good physical health and that she will notify staff members of any conditions that may impair her ability to participate in all camp activities.
