Pre-Participation Information - Freshman/Transfer Read the following rules to access ATS, Concordia’s electronic medical record:      

Do NOT use a Mac computer or Safari Web browser Do NOT use a tablet or iPhone Make sure your browser is in compatibility mode if using Internet Explorer Make sure pop up blocker is OFF (both on your browser and internet security) Make sure to sign the document at the end of all Athlete Forms and at the end of the New Athlete Health History form COMPLETION OF ALL FORMS WILL TAKE 45-60 MIN – SET ASIDE TIME. DO NOT RUSH.

1. Log on to- https://www.atsusers.com/atsweb/login.aspx?db=atscuchicago

* Put browser in compatibility mode (Internet Explorer only) and make sure your computer allows pop-ups.

2. First time log in –type new (case sensitive) for both Athlete ID and Password. 3. Select AGREE for the login agreement. 4. Athlete Information tab will appear. Answer completely (YELLOW highlighted sections are mandatory). a. Change your Athlete ID to your H# (example: H00123456), pick your own password (something easy you will remember). b. Select SAVE ATHLETE INFORMATION at the bottom 5. Once the Athlete Information tab is complete your page will look like this. Click on Athlete Information.

6. Click on the Medical History tab

a. Add any surgeries you have had. If none, leave blank. b. Skip the Medical Questions section until next year. c. Click on the Immunizations/Paperwork tab & add your Immunizations and Physical exam information. d. This tab also allows you to keep track of your completed Paperwork. e. Complete the Insurance & Contacts tabs THOROUGHLY. 7. Click on the Forms tab.

Click on the Form Name drop-down menu, select a form and click NEW. Click on each required form separately; selecting New each time. Read each form thoroughly; fill out each form completely and SIGN when done. To sign forms  click in the white box, hold down your left mouse button & move the mouse to sign.

e. You may also use your finger to sign. Type your name in the “Signed By” box then hit “Sign”. Click SAVE at the bottom of the page. f. Here is a list of the mandatory forms you must complete: i. Concussion Statement (Annually) ii. Consent to Treatment (Annually) iii. Drug Testing Consent (Annually) iv. Football Helmet Waiver (Annually) (Football/Men’s Lacrosse Only) v. Freshman/Transfer Health History (First Year Only) vi. Pregnancy Policy and Consent (Annually) (Females Only) vii. Sickle Cell Trait Waiver- (First Year Only) *read carefully and choose option 1, 2 or 3 (if unsure: look here  http://genes-rus.uthscsa.edu/sites/genes-r-us/files/state_contacts.pdf g. You WILL NOT practice until ALL FORMS are completed, signed & checked off by the Athletic Training Staff.

8. Next, click on the eFiles tab.

a. On the far right side of each page click View. (Some computers will prompt you at the top or bottom of the screen to OK the Download. Click on the bar and select download file, click

View again)

b. Print out the following blank forms: i. Physical Examination (Your Physical Exam MUST be conducted & signed by a Medical Doctor (MD or DO) – Forms by a Nurse, PA or Chiropractor will NOT be accepted. ii. Parent/Student Insurance Information (MUST be signed By BOTH you & one of your parents even if you are 18 years old or older). iii. Emergency Information (ATTACH a copy of the Front & Back of your Insurance Card &/or your parents card once the form is printed).

iv. ADHD Medical Exception (This is only required if you are diagnosed & currently taking medications prescribed by your physician/psychiatrist). c. Read and Fill out the eFile forms Completely, Scan and Upload them into ATS via the eFiles page. i. To scan: Set scanner to “MONO/BLACK” or 200 dpi/less. (The files cannot be bigger than 1MB.) ii. Scan as a PDF form (do NOT submit photographs taken with your cell phone). iii. Cougarnet (Concordia’s technology services), Kinko’s, or most libraries, schools and churches can scan these. iv. Save the scanned forms on a thumb drive/flash drive or have them emailed to yourself. d. When uploading your electronic document forms: i. Select eFiles. ii. Near the bottom of the page, type in a “Description” of the form you are uploading. iii. Click on “Choose File”. Find your form. Hit UPLOAD iv. Repeat steps until all forms are uploaded

9. After you complete a form, an email notification is sent to the Athletic Training staff. Allow the AT staff 48-72 hours to review and approve your forms. 10. Keep track of your checked-off forms under the Immunization/Paperwork tab. (Your head coach has access to monitor completed forms also.) 11.
