CUYAHOGA FALLS SPORTS PASS APPLICATION Our All-Sports Passes permit you to enter all home high school and middle school sporting events, excluding tournaments, OHSAA Tournaments and any event that Cuyahoga Falls may be hosting. Please fill in your information completely – we are making every effort to have the most up-to-date database of our supporters. All Sport Pass holders are required to have their picture ID on their pass. Pictures can be taken in the Activities Office Monday through Friday, 7:30am – 3:00pm.

Please note the following:  All-Sports Family Passes include two (2) adults and a limit of three (3) children. Additional student passes can be purchased for $25.00 per child (school age).  Please fill out the form at the bottom completely – provide first and last names of both adults and students being issued All-Sports Passes on the following page and submit with payment.  Replacement All-Sports Passes are $15.00 each.
