NOTE: DOORS CLOSE AT 7:00PM AND NO ADMITTANCE WILL BE ALLOWED AFTER THAT TIME

$15.00 per student Check, Cash or Money Order Payable to: CFHS
REQUIRED FORMS: OHSAA Pre-Participation Physical Evaluation Form:
Pages 1-5 printed off of Final Forms must accompany the athlete or Pages 1-6 printed off of Athletic Website, cfallsathletics.org, click on FORMS tab Signed and completed forms, as well as payment, MUST be turned in at time of physical.
PARTICIPANTS: Please come dressed for the exam. Flip Flops or take your shoes off Shorts; tank top or t-shirt
