REGISTRATION AND PARTICIPATION FORM FEBRUARY 25 MARCH 25 APRIL 8 Location: Bryant-Denny Stadium, Gate 32 (Indoor Studio) Check-in and Late Registration: 1:00p.m. -2:00 p.m. Dance Clinic: 2:00p.m.- 5:30p.m. Fee: $80 each clinic you attend (Checks —Payable to The University of Alabama) indicate which clinic(s) you will attend by circling date(s) above. Pre-audition dance clinics are offered to prepare dancers for Alabama Dance Team auditions or for anyone interested in auditioning for a college dance team. We will take you through the audition process to better prepare you for our auditions. The clinics are open to high school juniors and seniors, as well as college and transfer students. Pre-Registration • Complete and Return Signed Registration and Participation Form indicating which clinic(s) you will attend • Attach a copy of your medical insurance card (front/back)—required to participate • Include a check $80 (payable to The University of Alabama) • Mail to: Coach Morgan Williams, 1201 Coliseum Drive, Box 870393, Tuscaloosa, AL 35486-0393 CONSENT TO PARTICIPATE AND ACKNOWLEDGE OF RISKS I/We hereby acknowledge an awareness that participation in a DANCE CLINIC for the sport of dance involves risk of injury, paralysis, permanent mental disability, or death and that these injuries may occur in some instances as the result of unavoidable accidents. I/We accept these risks in giving consent to participate in the DANCE CLINIC at The University of Alabama. I/We also understand that a copy of CURRENT MEDICAL INSURANCE must accompany this participation form in order to participate in this DANCE CLINIC.
