Please fill out Allergies and Medications (Prescriptions) under the “Medical” tab of the “My Info” section of your athlete profile.
**MUST SUBMIT a copy of the FRONT and BACK of your insurance card, either uploaded or in person. **If you DO NOT have primary insurance, please type NONE in the Company Name.
HOW TO SIGN FORMS **Must print out the “Annual Physical Form” and have it filled out and SIGNED AND STAMPED by a Medical Doctor OR the Baruch College Student Health Center. **The two “Read Only” forms just need to be opened and read. It tracks if you have opened or not.
Once you “Select” and “Open” the document click on the “Save&Submit” button to electronically sign form
