Dodge County Youth Hockey Association EQUIPMENT LEASE 2011-12 Lease Agreement This lease agreement is between the Dodge County Youth Hockey Association (“Lessor”) and the noted (“Lessee’). The Lessor wants to lease hockey equipment to the Lessee, and the Lessee wants to lease hockey equipment from Lessor on the terms and conditions stated in this Agreement. Therefore, the parties make the following Agreement: 1. Lease: Lessor leases to Lessee and Lessee rents from Lessor the following items:

Term: The lease begins on the date of this Agreement and continues through September 1, 2012, subject to extension by agreement of both parties. 3. Rent: The rent for the equipment is $40 and payable in advance on the first day of the term of this lease. 4. Title: Title to the hockey equipment shall remain with the Lessor. 5. Warranty: Lessor, not being the manufacturer of the hockey equipment or the manufacturer’s agent, makes no warranty or representation as to the merchantability or fitness neither of the equipment nor as to the quality or capacity of the equipment, the workmanship of the equipment nor any other warranty whatsoever. All risks of the use of the equipment are to be borne by Lessee. Lessor is not responsible or liable for my direct, indirect, incidental or consequential damage to or losses resulting from the use of the equipment. Lessor acknowledges that any manufacturer’s and/or seller’s warranties are for the benefit of both Lessor and Lessee. 6. Use of Equipment: Lessee agrees to use the equipment only in connection with Dodge County Hockey activities engaged in by the Lessee’s child and shall not permit anyone other than Lessee’s child to use the equipment 7. Risk of Loss: Lessee assumes all risk of loss, damage, theft or destruction of the equipment. If such loss should occur, Lessee shall be obligated to Lessor for the entire replacement cost of the equipment. 8. Return of the equipment: At the expiration of the lease or earlier termination of this Agreement, Lessee will return the equipment to Lessor in the same repair or condition as at the beginning of the lease, reasonable wear and tear accepted. 9. Assignment: Lessee shall not assign the equipment to any other pain’ without Lessor’s prior written consent and approval. Additionally, Lessee shall not transfer or otherwise dispose of the equipment or lend it to anyone else. 10. This Agreement contains the entire Agreement between the parties and may not be modified, terminated, or discharged except in writing and this Agreement supersedes any and all prior agreements written or oral.
