1-2016-17 Parent Letter
Atlanta ISD Athletic Department May 15, 2017 Dear Parent/Guardian; Atlanta ISD athletes must have the following on file in the Athletic Department BEFORE participating in the 2017-18 Athletic Program: •
Pre-participation Physical Evaluation — All first- time participating athletes, all 7th, 9th and 11th grade athletes. o
The athletic physical includes a medical history and a medical evaluation.
o
The U.I.L. Pre-participation Physical Evaluation (2017) form provided with this letter must be used.
o
Athletic physicals can be obtained for $25.00 at Health Care Express, Hwy 59, Atlanta, for Atlanta ISD athletes.
o
Payment for the athletic physical is a parent responsibility.

Signed Acknowledgment of Rules — All participants
•
Signed UIL Parent and Student Agreement — All participants
•
Signed Acknowledgment Form For Anabolic Use and Random Steroid Testing — All participants
•
Signed Sudden Cardiac Arrest Awareness From — All participants
•
Signed UIL Concussion Acknowledgment Form — All participants
•
Signed Consent to Drug Screen— All high school students o
Consent to Drug Screen Form will be available at first scheduled pre-season football practice and during the schedule pick-up week
