Student Name (first and last) ___________________________ Your full name (first, middle, and last) will be used for the portrait picture. Throughout the rest of the yearbook, the editors like to use the “first name” most commonly used for you at school by students and teachers. Example: birth name: Alexander; most commonly called: Alex. First Name “School appropriate” nickname _________________________________
The yearbook editors would like to include a quote from each senior in the yearbook. The quote can come from various sources such as: your personal quote (self quote) existing quote (need to give credit to the original source which is sometimes anonymous): there are a couple of web page ideas on the next page quote from someone you know (need to give credit to the person the quote is from) If the quote is not acceptable according to the yearbook editors, advisor, and/or school personnel, it will not be placed in the publication. You will be given a one time opportunity to resubmit. Because of space limitations, it is requested to keep the quote at a maximum of 25 words. If it is too long, you may be requested to resubmit and/or the yearbook editors may shorten the quote. Give some serious thought to use a quote that reflects you. Legibly write your quote below OR attach a printed copy. Again, be sure to include the name of the person the quote is from.
