Dear Fairmont Senior Athlete, Attached is a packet containing the application for a Fairmont Athletic Booster Scholarship. Each year, the Fairmont Athletic Boosters award scholarships to graduating Fairmont High School seniors who have played a sport while attending Fairmont High School. This year, $10,000.00 is available and the intent is to award up to 10 scholarships, each worth $1,000.00. Winning students must be planning to attend a post-secondary school (trade, technical, college or university) after their graduation. The eligible students for this year’s scholarships must be graduating in May of 2015 and either the applicant or a member of his/her family must be a member of the Fairmont Athletic Boosters. Enclosed are the following forms: 1.

Be sure to allow enough time for your coach/advisor to complete the evaluation. Applications must be turned in to the Fairmont Athletic Office by 3:30 p.m. on Friday, March 27, 2015. NO LATE APPLICATIONS WILL BE ACCEPTED. NO EXCEPTIONS. The scholarships will be awarded at the Spring Scholar-Athlete Awards Program on Monday, May 4, 2015 at the Presidential Banquet Center. The Fairmont Athletic Boosters
Fairmont High School Athletic Booster Scholarship Award Program Purpose The Kettering Fairmont Athletic Boosters have established this award program to help finance the post-secondary education of graduating Fairmont athletes. Post secondary education can be defined as trade school, technical school, junior or community colleges or a four-year college or university. The intent of this program is to recognize outstanding athletic achievement, scholarship, citizenship, and leadership of senior student-athletes at Kettering Fairmont High School. This year, up to ten (10) one thousand dollar ($1,000.00) scholarships can be awarded. Scholarships will be awarded to seniors who have met the eligibility and selection requirements of the Boosters Selection Committee. Eligibility • Any graduating senior who has earned 1 (one) or more varsity letter awards in an interscholastic sport or in a related sports activity (such as cheerleaders, trainers, or managers). • All applicants must have participated in one (1) sports season during the senior year unless excused by a medical authority. If excused from senior year participation, the applicant must have participated in at least one (1) other interscholastic sports season during the junior year. • No applicant shall have been suspended from active participation in athletic or athletic support activities during the senior year. • Applicants must apply to and expect to attend a post-secondary educational institution (defined as trade school, technical school, community or junior college or a four-year college or university or other institution of higher education). Funds will be sent directly to the educational institution. • Applicants must complete an application and all required forms and submit them before the required date and time. • Applicant or a member of the applicant’s family must be a Fairmont Athletic Booster member. Booster memberships are available at a variety of levels of support. • Applicants are eligible for only one (1) Fairmont Athletic Booster scholarship per year. • In addition to the Coach or Advisor Evaluation included in this packet, the applicant must include at least one letter of recommendation from someone outside of the Fairmont Athletic Department (i.e. a principal, teacher, youth group leader, etc.) Note: Eligibility requirements may be waived or adjusted to accommodate special circumstances or special needs. A request for waiver of eligibility requirements (in the form of a short note or letter) must accompany the application. Selection Committee The scholarship selection committee will consist of five (5) members of the Kettering Fairmont Boosters including the committee chairperson. The committee members will be appointed by the President and presided over by a committee chairperson. An alternate may be appointed by the committee chairperson to replace a member who cannot complete his/her obligation. No member of the selection committee shall be a parent of a senior Fairmont student.
Selection Process • • • • • • •
Obtain an application package from the Athletic Office or download the application package at www.firebirdathletics.com. Complete the application (including essay) and turn in to the Athletic Office on or before the stated deadline. Early submittal is best. Applications will be verified by the Athletic Department. All eligible applications will be forwarded to the Selection Committee for further review. Selection committee members will grade the essay question along with the response to Section IV without knowledge of the name of the applicants. The results will be tabulated by the committee chairperson. Up to 10 applicants will be awarded the scholarships.
The committee is authorized to award up to 10 scholarships, if appropriate, but total expenditure cannot exceed $10,000.00. Award Distribution A letter and plaque will be presented to the scholarship recipients at the Scholar-Athlete Awards program usually held in May of each year. Disbursements of funds will be paid directly to the post-secondary institution. It is each recipient’s responsibility to present written proof of matriculation in a post-secondary educational institution to the Athletic Booster Treasurer. Photocopies of required tuition and/or other expenses will suffice as proof of matriculation. Deadline for awards collection is October 1 of the year of graduation from Fairmont High School. After this date, all unused funds will be returned to the Booster General Scholarship Fund.
