I am aware of the possible risks and hazards involved in such participation and I do hereby WAIVE, RELEASE AND AGREE to hold harmless the MGSL (its board members, directors, coaches, assistant coaches, organizers, volunteers, umpires and sponsors, including any one person and/or company whom supports this league (MGSL) by any means and other players and any others affiliated with MGSL AND the Marietta School System, which is not affiliated with MGSL but only are the owners of the fields at Phillips School, from any actual or alleged claims, demands, causes of action, liability, loss, damage and/or injury to property or persons arising out of any injury or death of said participant. This also allows MGSL to use your child’s name and/or any photos you child may be in, to be posted on the MGSL websites. Please contact a board member if their name or photo is not to be shown.
