2016 WILD CARD REQUEST FORM Wild cards are available for USTA NorCal Sectional Championships, USTA Zone Team Championships, USTA National Team Championships, and USTA Intersectional Team Championships. Wild Cards may be applied for in multiple divisions -‐ the oldest age division will be considered first. To be considered for a Wild Card, a player must submit a timely entry into the tournament and apply for a Wild Card no later than the tournament’s entry deadline by sending this request form to Tommy Tu at [email protected] or fax to (510) 748–7377. In the case of the national team tournaments (Zonals, National Team, Intersectionals), these are the dates by which you are required to submit an entry to the USTA NorCal office. This PDF form has been set up to allow you to type in all information other than the required signature using Adobe Acrobat Reader, Adobe Acrobat, or Apple Preview. If using Adobe Acrobat Reader, the typed information will not be saved when the document is closed; be sure to print before closing the document.
