2017 COUGAR TEAM FOOTBALL CAMP FOR HIGH SCHOOL ATHLETES
Thursday and Friday, July 20 and 21, 2017 Big Cat Stadium/Athletic Fields Directed by UMM Head Football Coach Rob Cushman and UMM Coaching Staff
Come test your team’s abilities on the turf at Big Cat Stadium. This two-day camp emphasizes individual development within a team framework. Players will receive individual position instruction from UMM staff each day followed by a variety of live competitions in the evening. Coaches will have the ability to observe their athletes, schemes, and strategies at work in a competitive atmosphere prior to the start of the season. A certified athletic trainer will be on site each day. This is a great opportunity to gain live experience before the 2017 season officially begins. For players in grades 9–12. Registration Camp Fee $35 per player; $650 team fee (20 or more players) (includes beverages and afternoon snacks each day and camp t-shirt) Head Coach signs up team (email camp coordinator Marty Hoffmann: [email protected]) Players register individually (see form on back; pay at the door) Register by July 14, 2017 Walk-ins welcome; $45 at the door Daily Schedule (Thursday and Friday) 3–4 p.m.......Arrive at Big Cat Stadium for check in/registration 4 p.m................................................................................Warm up 4:15 p.m................................... CAMP REGISTRATION, WAIVER AND RELEASE
I, the undersigned, wish to register my child, named above, as a participant in the above- named Camp, including related activities, and agree that: • The risks from activities involved in the Camp is significant, including serious injury and death; and while rules and equipment may reduce the risk, the risk does exist; • I knowingly assume all such risks, both known and unknown, and assume full responsibility for my child’s participation in the Camp; • I, for myself and my child, my heirs and next of kin, hereby release, indemnify and hold harmless Regents of the University of Minnesota, Morris Campus, the Department of Athletics, their regents, officers, officials, agents, employees, volunteers, Camp leaders and instructors, sponsors and advertisers (“Releasees”) with respect to any and all injury, including death, loss or damage to person or property, whether arising from the negligence of Releasees or not, to the fullest extent authorized by law. • I hereby authorize the Camp directors, instructors and staff to render first aid and/or obtain any medical treatment deemed necessary. I will be financially responsible for all charges incurred in the rendering of such treatment regardless of whether my medical insurance covers such charges. • My child has no medical condition or physical impairment that would inhibit active participation in the Camp. I will provide a list of medications my child takes, if any. *My child has the following medical conditions which may affect his/her participation in camp • I further agree that Camp directors, instructors and staff may photograph my child during Camp and that University retains the rights to use these visual images, with or without my child’s name, in any manner it wishes without compensation to me or my child.
