SUBMIT ALL FORMS WITH PAYMENT NO LATER THAN APRIL 11, 2018 BY MAIL OR MAILBOX DROP OFF AT PHOTO ARTS PRODUCTION STUDIO There will be 2 rehearsal dates at Nutley Parks & Recreation: Dates TBD Models MUST attend at least 1 rehearsal but are welcome at both. Photo Arts Productions Photograph Release Form: This release form gives Photo Arts Productions, Parks & Recreation, and the American Cancer Society permission to use the photographs taken at the fashion show. By signing this form you are in agreement that the photos taken for and at the Walking in Faith Fashion Show can be used for displays, websites, brochures, promotional material, etc.
