Apponagansett Farm 2016 CSA (Community Supported Agriculture) By signing this contract, I am choosing to become a member of Apponagansett Farm’s 2016 summer CSA. I understand I will receive a share of the harvest for 20 weeks, including the bumper crops and potential losses resulting from the beautiful yet fickle New England weather! I agree to pick up my share weekly at the time and location that I have selected or to make other arrangements in advance with the farmers. I understand that there are no refunds for weeks that I am away or if I choose to cancel my CSA membership (although you can transfer your share to someone else). I’m hungry for delicious, wholesome, local veggies. Sign me up!

□ PLYMOUTH – Shockyard Fitness and Social Club (19R Court St., Plymouth, MA) Pick up is on Saturday from 9am – 11am from early June through mid-October Choose Payment Option* (please enclose check with this form or pay online through website) □ Pay in full ($500) □ Pay $100 deposit to secure your spot and pay remainder by first CSA pick-up □ Pay $100 deposit to secure your spot and pay remainder through monthly payments during summer Optional Add-ons for Plymouth □ Egg Share ($120): 1 dozen eggs each week for 20 weeks □ Flower Share ($80): get a large bouquet of flowers each week during the summer for 10 weeks
□ DARTMOUTH - on farm pick-up (607 Elm St., Dartmouth, MA) Choose one day for regular pick up: □ Tuesday 3-7pm at the farm Choose Length of CSA
□ Friday 3-7pm at the farm
□ Full Season (20 weeks: June – October)
□ Short Season (10 weeks: July – August) Choose Share Size □ Regular Share (8 items per week; $520 for 20 weeks or $300 for 10 weeks) □ Large Share (12 items per week; $780 for 20 weeks; $450 for 10 weeks) Choose Payment Option* (please enclose check with this form or pay online through website) □ Pay in full □ Pay $100 deposit to secure your spot and pay remainder by first CSA pick-up □ Pay $100 deposit to secure your spot and pay remainder through monthly payments during summer *We accept SNAP benefits for full or partial payments and we have a few on-farm low-cost shares.
