Minneapolis Public Schools Eligibility Policy Any student wishing to participate in athletics should be making sufficient progress toward graduation (see chart below this section; each class is worth 0.25 credits) and have a 2.0 grade point average (G. P. A.) for the quarter prior to or during participation. If students fall below these standards they may be eligible for an appeal, see the athletic director to determine if you are meet the criteria for the appeals process. Student/athletes are expected to attend and actively participate in all classes! Grade

III. Pre-Participation Requirements All requirements must be completed before athlete can participate in a sport. No equipment will be issued until athlete is cleared through athletic office. Return all completed forms to athletic office. Please note that no insurance is available through the school. 1. Sports Qualifying Physical Exam No one will be allowed to tryout or participate on a team until she/he has a complete sports physical exam on file in the athletic director's office. This form must be signed by a doctor. NO EXCEPTIONS WILL BE MADE. The Minnesota State High School League requires physicals to be renewed every 3 years. This form is enclosed in this mailing. Please call Maryann Toledo at 612-668-1944 to set up an appointment at the school clinic. No walk-ins will be seen. Physicals are available all summer long. Physicals will also be available August 5th-16th in the School Based Clinic. As in the past all students must have a current signed parent clinic consent form as well as their athletic participation papers when they come for their physicals. They can get the parent consent forms and other paperwork from the clinics. 2. Parent Permission-Eligibility Form/ Health Questionnaire All participants must have a signed copy of the current Minnesota State High School League form on file in the athletic director's office. This form must be signed by the participant and the parent/guardian. This form is enclosed in this mailing. *Please note that additional copies of these forms are available on the Minnesota State High School League website. Go to www.mshsl.org , click on the Resources menu (top yellow bar), then under the Forms menu go to view all. The forms needed are Physical Examination Form (under Medical/Physical heading), and Athletic Eligibility Brochure 2016 - 2017. (under Publications heading) 3. Participation Fee The Minneapolis Public Schools Athletic Participation Fees are: Hockey - $90.00 Football - $70.00 All Other Sports - $60.00 There are pro-rated, reduced fees for students who qualify for free or reduced lunch There are no fee waivers and these are not refundable! No personal checks are accepted! IV.

Return of Equipment It is very important that all equipment be returned immediately at the close of each sport’s season. It is the student/athlete’s responsibility to return this. Parents and guardians have an obligation to see that all equipment is returned to equipment manager. Failure to satisfy obligations will be handled either administratively or criminally. Students will not be allowed to check out equipment until all obligations are cleared. If you have any equipment from previous years, please return it to the athletic director so your obligation can be cleared. V.

Ticket Prices Ticket prices are determined each year by the district athletic office. Prices for this year will be $6.00 for adults and $4.00 for students for conference games. Once you buy your ticket you may not leave the facility without purchasing another ticket. There are no in/out privileges and there are no ticket refunds. Discount punch cards are available, $35.00 for adults and $20.00 for students. These cards get you into 10 games at any Minneapolis Conference site. Realize that ticket prices for Non-Conference games and Section play-offs may vary! VII. Booster Club Patrick Henry has a booster club that supports all programs. Your help is needed to work at various fundraising activities. Please email Alana Strickler at [email protected] to join. VIII. Patrick Henry Clothing and Spirit Items If you would like to purchase memorabilia displaying the school name and logo, please contact the athletic department at 612-668-1947. The school’s booster club and the Patrick Henry High Foundation have various items are for sale including hats, shirts, sweat shirts, static cling stickers, pens, key chains etc. X.

Fun Facts about the Patrick Henry Athletic Program 1. Our school has a Hall of Fame which, is proudly displayed in our gym lobby. 2. Our school has produced many college and professional athletes (Too numerous to name!) 3. Our school has the best overall athletic facility of any Minneapolis High School, including baseball field with batting cages, football/soccer field, hockey rink, weight room, study room, indoor batting cage and 4 gymnasiums!

NCAA Eligibility and Athletic Scholarships The NCAA Division I and Division II initial-eligibility requirements have changed. All students who wish to participate in college must visit the website: www.ncaastudent.org and familiarize themselves with the information provided regarding participation in college athletics. It is required that students and parents read the Guide for the College-Bound Student-Athlete. This handbook is very helpful for those students and their families who plan to participate in college athletics. There are many details to qualifying for an athletic scholarship, please realize that these are very hard to receive. For more information please visit the NCAA web site www.ncaa.org. This is a wonderful web site with helpful information. Remember you must sign up for the clearinghouse if you are a prospective college athlete wishing to get an athletic scholarship. Also you must sign up for the ACT or SAT test early (Spring of your junior year or fall of your senior year). See your counselor or athletic director for additional information. 3
