Hall of Fame Nomination Form Deadline is March 1st of each School Year

Nominee’s Name _____________________________________ DHS Class of _______ (if necessary) A nominee for the De Soto High School Hall of Fame can be nominated in one or more of the following categories:

1. Academics   

De Soto High School Graduate Minimum of FIVE years out of high school Examples may include but are not limited to: National Merit Scholar Finalist, Valedictorian, scholarships, Perfect ACT/SAT, memorable academic accomplishments above and beyond the norm while attending De Soto High School

2. Activities /Athletics    

De Soto High School Graduate Minimum of FIVE years out of high school Team/Group or Individual Accomplishments Examples may include but are not limited to: All League, All Metro, All State, State Placer, Media Player of the Year, League, Regional, Sub-State, State Championships, 1st Chair, college success, pro athletic success, memorable accomplishments above and beyond the norm

3. Coaches/Sponsors    

Minimum of FIVE years coaching at De Soto High School, Head or Assistant coaching experience Coaching accomplishments Can be a current or former coach who is not currently coaching at De Soto High School Examples may include but are not limited to: Team Records, Coaching Record, Championships: League, Sub-State, Regional, State, Overall impact on students and/or school, etc.

4. Distinguished Service   

Loyal and devoted supporter of De Soto High School (DHS Alum Not Required) Exceptional character and role model for students and/or student-athletes (DHS Alum Not Required) Military and/or Public Service, Career Accomplishments (MUST be DHS Alum)
