Designation of Beneficiary All UNICEF staff members, regardless of category or duration of appointment, are responsible for designating a beneficiary or beneficiaries. They are also responsible for keeping this information up to date. In the event that the staff member dies while in service, the beneficiaries will be paid any corresponding entitlements. A number of different beneficiary forms are available for different entitlements: UN P.2 Form - Designation, Change, or Revocation of Beneficiaries Every staff member will be required to complete the UN P.2 form at the time of appointment. UNICEF requires the signed original version of this form, countersigned by a witness. In the event of a staff member’s death in service, any unpaid salary and allowances, as well as unused annual leave days will be paid to the beneficiary on this form. In case that the beneficiary on this form is not alive to receive the payment, or there was no designated beneficiary, the entitlements will be paid to the staff member’s estate. Please note that there are some entitlements which will be automatically paid to a surviving spouse and/or dependent children, regardless of whether they are designated or not as beneficiaries on the P.2 form. This includes, for example, the repatriation grant, the death benefit, or any entitlement payable under the Malicious Acts Insurance Policy (MAIP). UNJSPF A.2 Form – Designation of a Recipient of a Residual Settlement Upon appointment, staff members contributing in the UN Joint Staff Pension Fund will also be required to provide a signed original A.2 form. If a staff member dies in service and a residual settlement is due, it will be paid to the designated person(s) on the A.2 form. A residual settlement consists of the participant's own contributions at the date of his or her separation or death in service, reduced by the total amount of the benefits paid to and on account of the participant. Payment of a residual settlement to the designated beneficiary will not apply where there is a survivor (e.g. a dependent spouse or child) who is entitled to a monthly benefit under the Pension Fund regulations.
