A Separated Report has been requested which is defined as Section 1/Section 2 conditions evident on the date of inspection. Section 1 contains items where there is evidence of active infestation, infection or conditions that have resulted in or from infestation or infection. Section 2 items are conditions deemed likely to lead to infestation or infection, but where no visible evidence of such was found. Further Inspection items are defined as recommendations to inspect area(s) which during the original inspection did not allow the inspector access to complete his inspection and cannot be defined as Section 1 or Section 2. (1) Substructure Area 1A. There is evidence of drywood termites noted in the subarea, interior and exterior as indicated on the diagram. RECOMMENDATION: Due to the nature of these infestations, it will be necessary to seal the entire structure and fumigate it with Vikane (Sulfuryl Fluoride) gas, with tear gas (Chloropicrin) as a warning agent. The entire structure must be vacant for this process. We will use all due caution in our operation, however, we assume no liability for any damage to the roof coverings, solar panels, TV antennas or plantings adjacent to the structure. The occupants must sign the Occupants Fumigation Notice and comply with all of the instructions. NOTE: Instruction for preparation work will be forthcoming from the sub-contract fumigation company and must be complied with in order for fumigation to commence. Section 1 1B. Drywood termite pellets were found in the area(s) indicated on the diagram. RECOMMENDATION: Remove and/or cover the accessible drywood termite pellets. It is possible for old pellets to sift down and to become visible in previously infested areas due to normal movement and vibration of a structure. Our bid is for removing and/or covering the pellets on a one-time basis. Section 1

1C. Evidence of subterranean termites was found to be entering the structure from one or more of the following areas: subarea, exterior soil and beneath the concrete surfaces as indicated on the diagram. RECOMMENDATION: Treat the subarea soil with Premise (Imidacloprid). Drill a series of holes through all exterior concrete slabs, porches and walkways next to the foundation. Treat the soil with the registered material Termidor SC (Fipronil). Seal drilled holes afterwards. Remove all accessible evidence of subterranean termites as practical NOTE: If applicable, it may be necessary to remove deck boards where needed to access for treatment. Reinstall the boards upon completion of treatment NOTE: We assume no liability for damage that could occur to pipes and/or conduits that could be located under the concrete slab areas. It will be the owner's responsibility to provide J & T Termite with the necessary information as to the location of pipes/conduits under the slab prior to the commencement of this work. Section 1 1D. Subterranean termite infested cellulose debris was found in the subarea. RECOMMENDATION: Remove and dispose of the cellulose debris. Section 1 1E. Subterranean termite damage was noted to the cripple framing, mud sill and vinyl siding sheathing as indicated on the diagram. RECOMMENDATION: Owner’s to contact a licensed contractor to replace the damaged wood members as necessary. Section 1 1F. Surface fungus noted on the wood members in the subarea as indicated on the diagram. RECOMMENDATION: Scrape and treat the affected wood members with Tim-Bor (Disodium Octoborate Tetrahydrate). Section 1 1G. Drywood termite damage was noted to the subfloor as indicated on the diagram. RECOMMENDATION: Remove, replace and/or reinforce the damaged wood members as necessary. If damage is revealed beyond the visible condition, a supplemental report will be issued outlining any further findings and recommendations and an additional price will be quoted if applicable. 1H. Fungus damage was noted to the bracing below the master bath as indicated on the diagram. RECOMMENDATION: Remove and replace the damaged wood members as necessary. Section 1 1I. Subterranean termite damage was noted to the subfloor and floor joist as indicated on the diagram. RECOMMENDATION: Remove, replace and/or reinforce the damaged wood members as necessary. If damage is revealed beyond the visible condition, a supplemental report will be issued outlining any further findings and recommendations and an additional price will be quoted if applicable. Section 1 1J. Old dry water stains were found on the wood members beneath the kitchen area. We did not find any active leakage at the time of this inspection. Periodic inspections are recommended. Information (3) Foundation 3A. Minor foundation cracks were noted to the in some areas. RECOMMENDATION: For further information in regards to foundation conditions contact a licensed contractor to make a complete survey. Section 2 (4) Porches - Steps 4A. Fungus damage was noted to the railing as indicated on the diagram. RECOMMENDATION: Remove and replace the damaged wood members as necessary. This bid does not include painting. Section 1 4B. The rear step was inaccessible for inspection, due to lack of clearance. RECOMMENDATION: If desired or required a further inspection will be made after arrangements have been made by contacting our office. Further Inspection (6) Abutments 6A. 6B. The hollow abutment indicated on the diagram contain wood members for which we can make no representation regarding the presence or absence of wood destroying pests or organisms without installing test openings through the abutment for further inspection. A cost quotation for test openings for further inspection will be issued upon specific request. Information (10) Other - Interior 10A. The base of the hall bath vanity is fungus damaged. RECOMMENDATION: Remove the damaged vanity. Install a new vanity of standard paint grade. Plumbing changes are not included in this estimate. Section 1 10B. Drywood termite damage was noted to the window sill as indicated on the diagram. RECOMMENDATION: Remove and replace the damaged wood members as necessary. This bid does not include painting. If further damage is revealed, a supplemental report will be issued outlining further findings and recommendations. A bid will be issued for further repair if applicable. Section 1 10C. There are water stains under the kitchen sink counter top. RECOMMENDATION: Maintain sealants around the sink area as needed. Section 2 10D. A plumbing leak was noted below the kitchen sink. RECOMMENDATION: Owner’s to contact a licensed plumbing to repair the leak as necessary. Section 2 10E. At the time of this inspection repairs or replacements were being performed by others. No infestation or infection was visible and accessible in the exposed wood members at this time. We recommend that all repairs be completed in a proper workman-like manner to comply with all applicable building, health and safety codes. We do not guarantee any work done by others. Information

(11) Other - Exterior 11A. There is drywood termite and fungus damage noted in the wood member(s) located at the roof overhang as indicated on the diagram. RECOMMENDATION: Remove the roof covering as necessary to expose the damaged wood members. Remove and replace the damaged wood members as necessary. This bid does not include painting. NOTE: Our cost quotation does not include roof covering or rain gutter repair or re-installation. The owner is directed to contact a licensed roofing/rain gutter contractor to make any necessary repairs to the roof covering and rain gutter system after all necessary wood repair/replacement is completed. Section 1 11B. Fungus damage was noted to the window trim as indicated on the diagram. RECOMMENDATION: Remove and replace the damaged wood members as necessary. This bid does not include painting. If damage is revealed beyond the visible area, a supplemental report will be issued outlining any further findings and recommendations and a bid will be issued if applicable. Section 1 11C. Fungus damage was noted to the water heater closet door and siding as indicated on the diagram. RECOMMENDATION: Remove and replace the damaged wood members as necessary. Metal Z-Bar will be installed at the joints. This bid does not include painting. The exterior wood members of this structure were found to be concealed from the view of our inspector. This was due to vinyl siding being installed over the wood members. Areas Not Inspected: This report is limited to those portions of the diagrammed structure that were open and visible. We did not inspect any concealed areas to which there is no access without moving appliances, furniture, storage, floor coverings, insulation, cabinets, finished materials, etc. We did not water test and do not certify against leakage, any appliances, plumbing over finished ceilings, roof coverings or decks. We make no guarantee against infestation, infection, leakage, or any adverse conditions which may exist in such areas or which may develop or become evident in any areas after this date. If information about the areas not inspected is desired, we recommend further inspection be performed after they are rendered accessible by the owner. Guarantee: Fumigations performed by J&T Termite are guaranteed for three (3) years. Wood repairs and ceramic tile replacement performed by J&T Termite is guaranteed for one (1) year from the date of completion. The guarantee for localized chemical treatment is limited to the area(s) treated for one (1) year, except secondary substandard measurements which are not guaranteed. Vinyl floor covering, plumbing repairs, glass work, grouting, caulking or any measures for control of moisture completed by J&T Termite are guaranteed for ninety (90) days. Reinspection: This Company will re-inspect repairs done by others within 4 months of the original inspection. A charge, if any, can be no greater than the original inspection fee for each reinspection. The re-inspection must be done within ten (10) working days of request. The re-inspection is a visual inspection and if inspection of concealed areas is desired, inspection of work in progress will be necessary. Any guarantees must be received from persons or companies performing the repairs. Wood-destroying Organisms: This inspection is to disclose actual infestations, infections and conditions that are conducive to infestation or infection. This is not a general pest inspection (ants, spiders, cockroaches, fleas, rodents, etc.), but concerns only wood destroying pests and organisms and conditions conducive to such. Competitive Business Practices: The Structural Pest Control board encourages competitive business practices among registered companies. Reports on this structure prepared by various registered companies should list the same findings (i.e. termite infestations, termite damage, fungus damage, etc.). However, recommendations to correct these findings may vary from company to company. Therefore, you have a right to seek a second opinion from another company. Structural and Roof Inspection: This is not a structural inspection. If information concerning electrical, heating, foundation and any other conditions beyond the scope of a Wood Destroying Organisms inspection is desired, the appropriate inspecting firm should be consulted. The exterior surface of the roof was not inspected. If you want the water tightness of the roof determined you should contact a roofing contractor who is licensed by the contractor’s state license board. Plumbing: This firm inspects and reports only visible plumbing leaks on the day of the inspection. This firm does not guarantee against hidden or future leaks. The owner should contact a plumber for further information regarding plumbing. Mold: This property was not inspected for the presence or absence of health-related molds or fungi. By California Law, we are not qualified, authorized nor licensed to inspect for health-related molds. If you desire information about the presence or absence of health-related molds, contact the California Department of Health Related Services. Asbestos: No representation is made regarding the presence or absence of asbestos, a material commonly used in structures prior to 1983. If interested parties desire information concerning asbestos, a proper contractor should be consulted. Mill Patterns: It may not be possible to match the existing mill patterns and materials used in the original construction. We will replace damaged wood members with materials which resemble, as closely as possible, the existing wood members with standard grade materials currently in stock and available.

For further information, contact your health care provider or an industrial hygienist. A licensed pest control inspector is not an expert in lead, lead-based paint, or exposure to lead. This report is not intended to identify the presence or absence of lead or lead-based paint in the building inspected. Whether lead-based paint is present can be determined only by a certified lead inspector. Exterior: Where applicable the structure was inspected from the ground level only. Portions of the structure that extend more than 12 feet from the ground level were visually inspected only. Visual inspection above 12 feet is limited by height. Accuracy above this point cannot be guaranteed. Wood members showing visible signs of infestation or infection were pick-tested to determine the nature and extent of the finding. The inspector did not get up on the roof to inspect wood members accessible from the roof. Certification: Certification applies to visible and accessible areas of the structure observed at the time of original inspection. This certification does not constitute a guarantee that parties in interest will not inherit hidden infestations, infections and adverse conditions. J&T Termite assumes no liability for such hidden conditions. If a problem is discovered that was not disclosed during our inspection, do not disturb the area. Contact our office immediately to inspect the problem area and allow proper determination. Limitations: Inspection of fences, screeds and detached structures is not included in this report unless otherwise stated. In any slab floor areas or areas where siding extends below the exterior grade level, it is possible for termite infestations and damage to be present but not evident at the time of inspection. Our inspection of any insulated attic spaces, where applicable, is limited to viewing the area from the access hatch. If interested parties provide catwalks for sure footing and request further inspection, we will return to make a closer inspection of any attic space at an additional charge.
