DONATION REQUESTS The club receives over 300 requests for donations each year. To enable us to ensure the most deserving in the community are prioritised, please complete this form and provide any relevant supporting documentation. ELIGIBILITY FOR DONATION/SIGNED ITEM The following conditions apply: Must be within Western Australia. At least four week lead time from request to collection date. Excludes personal requests for birthdays, weddings or anniversaries. Requests for tickets are not considered unless under special circumstances. Charitable and non-profits are given priority. Successful applications will be notified via email. Payments to be made by credit card - and will only be charged if your request is successful. Pick-up for approved donation requests will be from the Perth Wildcats Head Office - WA Basketball Centre, 201 Underwood Avenue, Floreat 6014. Items can be posted as long as postage costs are borne by the applicant.
