Consent Purpose: The purpose of this survey is to collect feedback regarding your experience with ESS programs and services during the current semester/year. You must be 18 years of age or older to participate. If you agree to participate, you will be asked to complete a brief survey comprised of multiple choice and openended questions. This survey should take no more than 10 to 20 minutes to complete depending on the number of programs and services your are evaluating. Confidentiality: The information you provide is completely confidential. Individual participants will not be identified with their evaluation information. Data from this evaluation will be saved on the department’s secure drive. Voluntary Participation: Your participation in this survey is voluntary. You may choose not to participate.
