I hereby waive and release Ennis High School and all clinic staff from any and all liabilities for injury or illness incurred while participating in this clinic. I give the clinic staff permission to act for me according to their best judgment in case of an injury.

INFORMATION SHEET The ages for the campers will range from age 8 through students having completed grade 8. U.I.L. restricts anyone in high school from participating. The camp fee is $50.00 per camper. Payment is due at time of registration. Early registration is appreciated! Please order your T-shirts larger than needed to avoid sizing conflicts. Camp will run from 8:30-11:30. Parents can drop off the campers at 8:15 for roll check and organization at the baseball fields 1st base dugout. Campers will meet in the dugout every day at 11:30 and be released to their parents. Please come in the stadium area to pick up your child. This will help alleviate safety issues in the parking lot
