Flowing Wells Continental Little League Agreement 2018 Please read and initial the following statements. Specific details can be found in the FWCLL Constitution and Bylaws. Upon request, copies are available for free in email format or on our website. A copy is also available for reference in the snack bar or from your team manager. ___________ I understand registration fees include but are not limited to: the league charter, insurance for player, light usage, league equipment, umpire fees and a uniform that includes a hat & shirt. ___________I understand that I am responsible for purchasing additional uniform items including but not limited to: baseball/softball pants, belt & socks and protective gear. ___________I understand that FWCLL is a volunteer organization and that I might be asked to help volunteer by the manager of my child’s team to help support the team and/or the league. ___________I understand that I will be asked to support the FWCLL fundraiser. I will be given the option to “buy” out of participating in the fundraiser. This fundraiser and the buyout fee will be per player and not per family. ___________I understand I may be asked for monetary donation aside from registration fees, to purchase a team banner or other team related items. Additional funds may be solicited depending on the team my child is placed on. I understand that not every team will ask for these. ___________I understand that the use of all our home parks for practices and games is a privilege, that can be suspend by Pima County or the City of Tucson at any point for failure to keep them maintained. To help FWCLL maintain the uses of these parks, I will assist my player’s team with field maintenance before and after practices and games. I will also assist with making sure that dugouts, bleachers and surrounding areas are cleaned of any debris. ___________I have read and agree to the terms of the Parent Code of Conduct. I understand that I can be removed from ANY Little League Event for violating the terms of this Code of Conduct. ___________I have provided FWCLL with the necessary paper work to register my player. I understand that my player will not be placed on a team until their registration packet is complete which includes a registration form, medical release, parent & player code of conduct, copy of birth certificate and either 3 proofs of residency or school enrollment. ___________I understand that all FWCLL General Board meetings are open to everyone and I am encouraged to attend to keep apprised of what is happening in the league. ____________I understand that it is my responsibility to inform a board member of any improper conduct by coaching staff, umpires, other parent or board members. Feedback is important when assessing the quality of our volunteers and staff to ensure the well-being of the players. We rely on you to report any serious issues, infractions and to share constructive comments. This feedback needs to be in written form so it can be handled appropriately. Forms are available at the snack bar, a board member or our website at www.fwcll.com. All reports are kept confidential.
