FOREST HILLS YOUTH BASEBALL MANAGERS CODE OF CONDUCT AND RESPONSIBILITY FORM By executing this form I hereby acknowledge that I have applied for, and have now been approved by vote of the Board of Directors at Forest Hills Youth Baseball, to serve in the role as a Manager of a team for the Spring Season 2016. By agreeing to volunteer to serve in the position as Manager of a team at Forest Hills I also understand and agree that I am the designated individual that is primarily responsible for the health, safety and enjoyment of the players on my team as well as the compliance with all applicable rules governing Forest Hills Youth Baseball, and any and all park/league regulations and protocols including, but not necessarily limited to, the following: Safety, Code of Ethics and Conduct In addition to the Coaches Code of Ethics Pledge Form I have previously executed I also understand and agree that I am the individual designated to be responsible for ensuring the safety and well being of my players during all league approved and scheduled practices, games and league approved/sanctioned activities at the Park. I understand that by virtue of my acceptance of this responsibility as the team manager I agree to the following: 1. For the safety of the players, parents and coaches I will strictly adhere to and enforce the Forest Hills Youth Baseball policy for severe weather and lightning. If at any time during a practice or game there is a lightning strike within 10 miles of the park I will immediately remove all players and coaches from the field and dugout. I understand that there is a mandatory 30 minute wait period that must be observed and that no player or coach will be allowed to go back onto the field to continue any practice or game until no less than 20 minutes has passed without another lightning strike within 10 miles of the park. 2. As manager I am ultimately responsible at all times for the conduct of the players, coaches and parents of players on my team during any scheduled practice or game. I agree to immediately address or otherwise take all necessary action to ensure that all players, coaches and parents of players on my team conduct themselves in a positive and courteous manner with good sportsmanship at all times. If there are any problems or incidents that occur during the course of any practice or game, I understand that I am responsible for promptly reporting any problem or incident to the designated Player Agent in my division or the appropriate member of the Board of Directors. 3. As manager I have the responsibility to immediately report any injury to any player or coach that occurs during any team practice or game. Any injury that is to be reported shall be

promptly communicated, in writing, to the Player Agent in the division and if necessary, the Senior Player Agent/Board of Directors. The reporting of all injuries sustained by any player or coach is to be completed using the league approved form available on the league website. I also understand that per league rules I am responsible to promptly notify the Player Agent in my division, in writing, if any player on my team quits the team for any reason or is absent for two (2) consecutive games. Field Maintenance and Care 1. I understand that as manager I am responsible for preparing the field for all practices and games for my team. Specifically, per the park requirements and protocol, I am responsible for ensuring that the field is dragged after each practice on a weekday night or if my team has the last practice or game on the weekend. It is also my responsibility to ensure that the field is left with all tarps replaced and properly locked, with all field maintenance equipment returned to the storage shed with that shed also properly locked. I also understand that I am responsible for attending (or having a designated coach attend in my absence) all league required “work parties” for field maintenance and up keep. Finally, I understand that it is my responsibility to prepare the field for play for any league scheduled game. 2. I understand that I am responsible for ensuring that all trash, debris, etc., is removed from the field and dug out after the completion of all team practices and games. Additionally, if my team is the home team for any league games I understand that I am responsible for ensuring that the remote control for the scoreboard and official scorebook are returned to the Board room at the park immediately following the game. General Rules and Responsibilities 1. I understand that in compliance with the league rules and protocols for the Spring 2016 season, there will be NO RESCHEDULING of any games for any reason other than inclement weather without prior approval of the Board of Directors. Due to the number of teams and limited field availability this season any request to reschedule a game absent extreme exigent circumstances will not be approved by the Board. 2. I understand that my team is to practice only on designated field and cage practice times as approved by the league, Board of Directors and the Player Agent in my division. If I have a request for any additional practice time outside of the scheduled league practices any such request must be communicated to and approved by the Player Agent in that division. No Manager or coach is authorized to schedule any team practice at a different time, a different field at the park or at an off site location without notifying the Player Agent and obtaining the Player Agent’s approval.

3. I understand and agree that all discussions, transactions and league business that transpires during the process of the league draft or All Star Selection process for which a manager is present is intended to be private and confidential, to the extent possible. There are to be absolutely no discussions or communications of any kind regarding the details of the draft or All Star selection process beyond the Managers (or other designated team representative) and the Board Members in attendance. 4. It is understood and acknowledged by me that the individuals selected to manage or Coach any league approved All Star team will be chosen and approved by the Board of Directors in its sole discretion. A manager does not automatically earn the position as All Star manager as a result of winning the division championship although that is one of the criteria that will be taken into considered by Board of Directors in the selection process.

I understand and acknowledge that by executing this form I am agreeing to strictly comply with all of the above stated requirements as well as all rules applicable to Forest Hills Youth Baseball. Any failure to comply with the above stated requirements contained within this Managers Code of Conduct and Responsibility Form, the Coaches Code of Ethics Pledge and all applicable rules for Forest Hills Youth Baseball may subject me to possible discipline including, but not limited to, suspension for a designated period of time (i.e. such as a specific number of games or practices), disqualification from managing or coaching a team (either during the season or for future seasons), or in more extreme cases removal from participation in Forest Hills Youth Baseball. All violations or other disciplinary matters will be handled and addressed by the Board of Directors in compliance with the league by laws and the applicable rules of Forest Hills Youth Baseball.
