To be accepted by Hawthorn Football Club this HawksDirect opt-out form must be completed in full with all contact information and all members who are opting-out of the payment plan

This form must be mailed or emailed to the Membership department. No opt-out forms will be accepted by phone or fax
•
Any monies paid on HawksDirect prior to Hawthorn Football Club receiving this form will not be refunded if the membership has been used to attend matches
•
Members seeking a refund of instalments must return their core membership pack and membership card to the Club before a refund is processed. Items can be returned to:

Please include your full name and member ID with the returned items

If there is an outstanding amount owed from previous years of membership, this amount must be cleared prior to re-joining as a member of Hawthorn Football Club
•
The submitting member must seek permission from all members included on the form, unless the submitting member is a parent/guardian of the member
