Application Due into Henry County Soccer Association by February 28 Open to graduating high school seniors in the Georgia public school system, private school system or school of independent study who have participated in a HCSA recreation or club soccer program for three seasons including the current Fall and or Spring seasons. The scholarship is in the form of tuition awards in the amount of $1000 to be used toward expenses in an accredited college, university or community college in the fall of the year awarded. Payment shall be made directly to the university financial department. The scholarship is based on the applicant’s academic achievement, soccer participation, written statement, references, and community service activities. The Henry County Soccer Association advertises the scholarships to age eligible soccer players and their coaches in the Henry County Soccer System. HCSA Scholarship Committee representatives will review all applications. If necessary, personal interviews are conducted with each finalist. Finalist must be available on the scheduled interview day to be eligible to receive a scholarship. Application Requirements Checklist Only complete application packets will be accepted (all parts must be submitted in one package prior to the deadline). You may apply for this scholarship before you have committed to a college. Awards are contingent upon proof of acceptance/enrollment in an academic or vocational program at an accredited four year college, university or community college with a minimum course load of 12 units or hours in the Fall of the year the application is placed. 1.) Complete all pages of the application form. 2.) Provide a copy of your official high school transcript. 3.) Provide applicant’s written statement outlining what soccer has meant to you, your soccer background, your personal goals/objectives for college, and your career ambitions. Applicant may also include additional background information (not listed on application) 4.) Letter of recommendation from applicant’s school soccer coach and/or HCSA soccer coach. 5.) Letter of recommendation from applicant’s faculty advisor or teacher (other than athletic). 6.) Letter of recommendation from a community service organization or volunteer who can describe the applicant’s community/volunteer service. 7.) Separate listing of your soccer volunteer and community service activities, the name of the organization, dates and what your activities included.
Certification I certify that all of the information I am submitting is true and accurate. I understand that all information must be submitted in “One Package” at “One Time” by February 28, to be considered. Scholarship payment will be sent to your college before July 31.
