Hilliard Ohio Soccer Association Financial Aid Application HOSA is a nonprofit organization and has limited amount of funds to assist those in need. If you are interested in applying for financial aid please fill this out and mail the completed form to HOSA, P.O. Box 1, Hilliard, Ohio 43026. Once your application has been reviewed you will be contacted by HOSA in a timely fashion. One scholarship per family is awarded. Deadline for financial aid applications: must be emailed or postmarked by closing date of open registration of the program you are th th applying for . This must be received by the deadline of June 30 for Fall season and January 30 for Spring season. No application will be considered after the deadline.

I understand by requesting financial aid from HOSA that I will need to divulge family information and reason for my financial need in order to be considered for a scholarship. Recreational (HRSL), PK Paws, and Jr. Academy: -A $30.00 application fee is required for U5 Rec (HRSL) and PK Paws for the child you are applying for and is due at the time the application is made. If financial assistance is granted the $40.00 application fee will be applied to the player’s share of the registration fee. -A $40.00 application fee is required for U6-U12 Rec for the child you are applying for and is due at the time the application is made. If financial assistance is granted the $40.00 application fee will be applied to the player’s share of the registration fee. -A $55.00 application fee is required for Jr. Academy players applying for financial aid and is due at the time the application is made. If financial assistance is granted the $55.00 application fee will be applied to the player’s share of the registration fee. -Any balance due must be paid within 10 days unless other arrangements have been made. Discounts for Recreational, PK Paws, and Jr. Academy: Recreation (HRSL) and PK Paws up to $15.00 discount for U5, up to $30.00 discount for U6-12 Jr. Academy

MSSA and HSSA -Both require a $80.00 application fee is required for the child you are applying for and is due at the time the application is made. If financial assistance is granted the $80.00 application fee will be applied to the player’s share of the registration fee. -Any balance due must be paid within 10 days unless other arrangements have been made. Discount for MSSA/HSSA: up to $40.00
Hilliard FC, HFC Select, and U8 Academy -The deposit must be paid in full before a financial aid discount can be applied. Discount for Hilliard FC and U8 Academy: Hilliard FC up to $125.00 discount for blue/royal teams, up to $100.00 discount for white team HFC Select:
up to $60.00 discount for U9-12
