Internet Permission _____ I understand that Impact FC (IFC) and/or an individual team may publish my child’s photograph and/or first and last name via the club’s websites, individual team websites, Premier UK (PUK) website or Soccer In College (SINC) website. _____ Therefore, I agree to indemnify, defend and hold harmless the members of IFC, its officers, coaches, assistants, managers, staff, parents and any and all other assignees from and against any and all claims and liabilities form this publishing. Permission is granted for the uses requested above. Membership Agreement with Impact FC _____ The IFC Code of Conduct form has been read and we agree to comply (parents and players) with these expectations and be subject to discipline for violations determined by IFC. A copy of the Code of Conduct is on the back of this form. _____ I understand the commitment to IFC and my team is for one year. Membership is for one year beginning on June 1st. I understand that if I choose to leave IFC, I am responsible for all club, team and training fees and that my child will not be released to play for another club until fees are paid in full. In addition, I understand my child will not be eligible to try out for any IFC team until overdue fees are paid in full. Financial Agreement Club Fees are paid directly to IFC to cover expenses such as, but not limited to, field rentals for training, player registration and office administration. These fees are set by IFC based on the various levels of play offered at IFC. All IFC players much purchase a uniform kit, which run on two-year cycles. Prices and details for the uniform kits will be provided via email or on the club’s website. Team Fees are separate expenses paid directly to your assigned team. Team Fees cover expenses such as tournaments, referee fees, coach’s travel, field rentals for games, etc. These fees will vary from team to team and are based on the amount of tournaments and travel your team participates in. All expenses to travel with the team and to/from practices, scrimmages, games and tournaments are your responsibility. Need based scholarships are available but are not guaranteed. The availability of funds is dependent on the success of club fundraisers which vary from year to year. Players must apply for need based scholarships. If awarded, it is NOT applicable toward commitment fee. Agreement By signing this contract, you acknowledge you have read and understand this form and agree to the following terms and conditions: The commitment fee is non-refundable. Upon commitment to a team, you are financially responsible for all Club Fees and Team Fees as stated for the entire playing year.
Beginning May 1, 2015 players will not be released from IFC unless all Club Fees are paid in full and Team Fees are up to date at the time of the request. Requests for releases and withdrawals must be presented in writing to your team manager, who will forward the request to the IFC Board for consideration. All fees will be paid when payment is due. Any payments that are delinquent may be cause to suspend a player from practices or games. Returned checks are subject to a $30 fee. IMPACT FC CODE OF CONDUCT
By signing the Intent To Play and Financial Agreement, I/We the parent(s)/guardian(s) of the

, agree to the following code of conduct: I will conduct myself in a reasonable and responsible manner as a parent representative of Impact FC and a role model for my child. I will respect my team’s coach and abide by his/her direction and leadership I will respect the opposing team and its players and will not engage in any disrespectful conduct. I will respect referees and will not challenge or show dissent with their officiating. I understand that I am also responsible for any non-parent spectators that may be with me.
If I violate any of the above provisions, I will adhere to the following discipline: Disrespect to an opposing player: 1 game suspension Ejection from a game (first offense*): 1 game suspension Ejection from a game (second offense*): 1 season suspension Ejection from a game (third offense*): Life time suspension from participation in Impact FC / ICYSA / ICRD programs including both recreation and competitive levels. Verbal assault/hostile challenge of a referee/coach/player: 1 year suspension from attending games. Physical assault of referee/coach/player: Life time suspension from participation in Impact FC / ICYSA / ICRD programs including both recreation and competitive levels.
*This code of conduct does not exclude discipline by the NCYSA. In the case of an ejection, the parent must leave the park/facility (out of sight and sound) in which the field is located and cannot return until 1 hour after their team’s next match. If the parent returns before this time, an additional 2 game suspension will be incurred.
