Commitment to the Sikh faith An assessment and evaluation under Criterion 2 of the Admissions Policy. The Religious Questionnaire (RQ) must be fully completed. The form enables the Trust to assess your commitment to the Sikh faith. The Trust will be making the assessment, categorising and grading the forms. Any information which is incomplete or not submitted will jeopardise your chances of obtaining a place at the Khalsa Secondary Academy School. The evidence provided is confidential between the parent and the Trust. The Trust only informs the school of the outcome of your Religious Questionnaire grading as a mark indicating Good, Satisfactory or Insufficient as explained fully in the Admissions Policy. They do not share information about your religious beliefs.
Please remember that in the event that fraudulent information is given in the application/questionnaire, the offer of a place would be immediately withdrawn and may result in the removal of your child from the school, in accordance with the School Admission Code.
The completed form should be sent by recorded delivery (one that guarantees delivery and records the name and address of the sender, date posted and delivery address) to:
