La Mesa National Little League 2017 Payment Plan Application INSTRUCTIONS La Mesa National Little League is committed to providing a program that allows every child to play baseball. Our mission is to offer an affordable program for our players and families while maintaining the financial health and sustainability of the league. Payment Plans are available to families that request them. Because LMNLL funds are based on community sponsorships received, and the overall number of registrations, payment plans are limited, and MUST be PAID IN FULL prior to any TOC or ALLSTAR participation. Applicants may be asked to provide additional information to prove the player is eligible for a payment plan. Please do not ask for a payment plan if you have the ability to pay in full at the time of registration. This application is entirely confidential and will only been viewed by the league President and Treasurer.

1st Payment (1/2 of Reg Fees) Due at time of Registration, 2nd Payment (1/2 of Reg Fees) Due February 20th 2017

4 Payments -

1st Payment (1/4 of Reg Fees) Due at time of Registration 2nd Payment (1/4 of Total Reg Fees) Due: February 20th 2017 3rd Payment (1/4 of Total Reg Fees) Due: March 20th 2017 4th Payment (1/4 of Total Reg Fees) Due: April 20th 2017
