Letter of Commitment I, _________________________________________, do hereby accept a position at Central Connecticut Youth (Players Name) Hockey Association on the ______________________________________________ Team for the 2016-2017 USA Hockey and Connecticut Hockey Conference Ice Hockey season. We, the undersigned player and parents/legal guardians of said player, by executing this Letter of Commitment, and paying the $300.00 Commitment Fee, agree to the following terms and conditions of this commitment: 1.

We agree to remain with Central Connecticut Youth Hockey and skate on the team listed above for the entirety of the 2016-2017 season. We understand that CCYHA reserves the right to move any player prior to October 1st based off an incorrect placement during the tryout process which may result in moving up or down a team within an age group. We understand that CCYHA reserves the right to add players in any position to rosters as it deems necessary in accordance with CHC and USA Hockey Policies and Procedures. We understand and agree that CCYHA’s obligation is to provide qualified coaches with the appropriate certification levels. Commitment is to CCYHA as an organization and not to a specific Coach. We agree to pay in a timely manner the full season financial obligation for the program and team listed above. We understand and agree that there are no refunds and that the fees for the entire season are due and payable in accordance with CCYHA’s 2016-2017 Payment Policy & Fee schedule upon signing this letter of commitment. Except as set forth below, we understand and agree that if the above named player refuses to participate with CCYHA after signing this agreement that he/she is ineligible to participate with any other Connecticut Hockey Conference member program and must sit out the remainder of the 2016-2017 season. We understand and agree that a player who sits out due to refusal to participate remains obligated to satisfy any and all outstanding financial obligations due to CCYHA for the 2016-2017 season. Except as set forth below we understand and agree that a player who sits out all or a portion of the season is not entitled to any partial or full refund or forgiveness of program fees or expenses. We understand and agree the above named player must obtain a proper Connecticut Hockey Conference release from CCYHA, and satisfy any financial obligations to CCYHA and return all CCYHA property, prior to committing to any new program in any ensuing season. We agree to abide by all Rules and Regulations, Code of Conduct, Policies and Procedures and Zero Tolerance policies, as set forth by CCYHA, The Connecticut Hockey Conference and USA Hockey. Failure to do so could result in suspension or expulsion from participating in any and all CCYHA Practices, Games, Tryouts or Events as either a player or spectator. We also recognize that we are not entitled to any Fee or Tuition refunds as a result of disciplinary actions taken by the Board of Directors for CCYHA. CCYHA understands and agrees that if the above named player is accepted to play on and commits to a Tier 1 program within the Connecticut Hockey Conference this contract will be void and the commitment fee will be returned in full. The $100.00 tryout fee is non refundable. We recognize that Hockey is a dangerous sport and injuries could result. We also recognize that due to the length of the season, illness can also occur that could result in missed practices and game. We understand that we are still obligated to satisfy any and all financial obligations regardless of the amount of time missed. Any waiver of fees is at the discretion of the CCYHA Board of Directors in accordance with the CCYHA Payment Policy and Procedure. We agree that if we are to accept a position on a FULL year Bantam Team and we choose to leave mid-season to play on a High School team we are responsible for paying the full year tuition for that team. Any waiver or discount is at the discretion of the CCYHA Board of Directors. ONLY THOSE PLAYERS WHO HAVE DECLARED HALF YEAR ELIGIBLE WILL BE ALLOWED TO LEAVE WITHOUT BEING RESPONSIBLE FOR FULL YEAR TUITION. BANTAM PLAYERS ONLY – By checking this box, I am declaring my intentions of trying out for my high school team in the fall. CCYHA will guarantee you a spot on a travel team if you do not make your high school team. Only players checking this box will be eligible to leave mid-season without being obligated to pay full year tuition I/We agree to release, absolve, indemnify and hold harmless the CCYHA Sponsors, Board of Directors, Coaches, Managers, and Supervisors (collectively, the “Indemnified Parties”) for all injuries to my/our child except to the extent of the gross negligence or intentional misconduct of the Indemnified Parties I/we do hereby waive all claims against the Indemnified Parties injuries incurred during practices, games or other activities except to the extent of the gross negligence or intentional misconduct of the Indemnified Parties.
We, the undersigned player and parents/legal guardians further understand and agree that this Letter of Commitment may only be terminated by mutual consent between the above named program and player/Parent/Legal Guardian. Upon such mutual consent, and payment of any outstanding financial obligations, the program will issue a proper Connecticut Hockey Conference Release and the player may transfer to another program. Commitment to CCYHA involves a number of steps. First, we would ask all members to complete the CCYHA Commitment Agreement. Secondly, we would ask that you complete the USA Hockey registration requirement. Lastly, we would ask that all members process the $300 commitment fee via the Central on-line registration system. These steps must be completed by April 3, 2016. Commitment Requirements 1. Execute the Commitment Agreement and send/bring agreement to the CCYHA Registrar (Aaron Odell). 2. Process payment for $300.00 via Central’s on-line registration system. 3. Complete the USA Hockey registration and send/bring your confirmation to the CCYHA Registrar.
USAH On Line Registration
A Step by Step guide to Online Registration Online registration for the 2016-2017 season is currently open. If given the option, make sure you ONLY REGISTER FOR THE 2016-2017 SEASON. 1. Go to http://www.usahockeyregistration.com/ 2. Choose member type "Ice Player & Coach". Be Careful NOT to select “Inline Player & Coach” this will not be accepted. 3. You must be 18 years of age to process a registration. Check the box that acknowledges that YOU are 18 or older. 4. Select the correct season (if prompted)– "2016-17". 5. Follow the prompts…fill in all data correctly. Use Names that are listed on your Birth Certificates.... No Nicknames. COACHES – DO NOT COMPLETE AN ON LINE REGISTRATION. CCYHA WILL BE DOING THIS FOR YOU. 6. Process payment $40.00 – Visa, Master Card, Discover or American Express (credit or debit card). This covers you USAH Dues and your CHC dues for the season 7. Print your Confirmation page at this time and make sure it includes the Bar code. It will also be emailed to you immediately after the registration is completed. Each participant will pay the USA Hockey fee of $30 if age 7 or older. The USA Hockey fee is waived for age 6 and younger but the registration must still be completed to get the confirmation number. Participants will also pay any applicable Affiliate fee. In Connecticut you will also pay the $10.00 CHC dues. If a participant plays and coaches he/she pays only one registration fee. Rev: 3/1/2012
Additional information: The home address should be used for all participants. If a player lives in one state and plays in another, enter the home address and pay any applicable Affiliate fee. The confirmation page/number will be accepted by any USA Hockey program in the country. Need a computer? Try your local library or school. A Visa gift card can be used for payment if no credit card is available. An email address is required. You can use a friend’s address if you don’t have one.
Once you have completed both your commitment agreement and your USAH On-Line Registration. Please send or email the following to the CCYHA Registrar: 1. Signed Commitment Agreement 2.
