Awards and Recognition Portfolio This booklet is designed to help organize your accomplishments - this is your time to shine. Please include everything you have done in 4-H and your community. It is recommended to include pictures of you doing project work and other things you are proud of. Don’t be shy ask your parents and/or leader to help you. Don’t forget to include a recommendation from your leader, other adult or parent. Plan to attend the Awards and Recognition Banquet in January. 1. The new 4-H Portfolio includes the trip application form and explanation. Be sure to use the form dated revised October 2016. and remember - if you want to try for a trip you MUST fill out the page 11. 2. Answer each question in the 4-H portfolio and tell your 4-H story. You might find your 4-H scrapbook helpful. If you don’t have a scrapbook ask your parents and leader to help you remember and make a mental note to start a scrapbook. a. Page 4 must be filled out completely. b. 4-H project summary tells the reviewer what you did. Brag a little - it’s ok. c. 4-H activities & trip section is where you tell about all the fun things you have done in 4-H, include as many as you can think of. d. 4-H presentations and communication section tells the story of your presentation career. Remember to include club and county presentations. Also any other opportunities that you had to speak in front of people as a result of being a 4-H member. e. 4-H leadership section is an opportunity for you to share the ways in which you used your leadership skills. Trade Show f. 4-H community service section tells the committee the story of how you con tribute to the community. What has your 4-H club done? What have done on your own?

Home, school and community section is another opportunity to brag to the committee on what you have learned in 4-H and how you have applied it in your community. What have you done at school, church or other community organizations? h. What 4-H means to me section is the most important or memorable thing you have done in 4-H this year. i. Award trip section is an opportunity to apply for a trip of your interest. If you are not interested in a trip - write that on page 11. 3. Thank goodness—you’re done. Make sure your portfolio is neat and readable. Now that you did all this work you want people to be able to read it. Many of your experiences may overlap within sections, that’s ok. How did you feel after an event or when you completed a project? Tell us about how it feels in each of the sections. 4. Be sure to turn your completed portfolio in by November 1. The committee needs time to review your work. This portfolio should reflect you and your experiences. Each year of your 4-H portfolio should be separate. This portfolio is an ongoing record of your 4-H experiences and accomplishments. Sam Filler, Development 5. It is better if you turn your portfolio in person but if you can’t call us for special directions for filing electronically. If you have applied in previous years, you should fill out a new form this year and submit it in a three ring notebook with the forms from previous years to make a complete record. Award Trip Section Guidelines: 1. All teens, currently enrolled in 7th—12th grade with at least 1 year or more of club work are eligible to apply for the following trips with consideration to age appropriateness considered. 2. All trip winners will be announced at the Awards and Recognition Banquet. Further trip details will be sent closer to the time of the trip. 3. Be sure to fill out the trip form as completely as possible and return to the Cooperative Extension Office with your 4-H Portfolio. 4. If you are not interested in a trip please indicate that on the form and include in your portfolio. Capital Days Objective: Participants gain insight into how State government works, explore career opportunities in government and meet with legislators. Great opportunity to meet teens from across the state. Sam Filler, Empire State Development When: May (Sunday—Tuesday) Where: Albany, NY Age: 9th-12th Grade Cornell Career Explorations Objective: Gain insight into career and job opportunities. This trip allows for an opportunity to experience Cornell University campus. When: Last week in June Where: Cornell University Age: 7th-12th Grade

Continue Award Trip Section Honor Trip Scholarships Receive a scholarship to be used towards the educational experience of you choice. The choices will include:
A.B.C. Trip—End of September to early October trip to SUNY Cobleskill explores numerous career opportunities in the agricultural field. STARR—This weekend offers an opportunity to join with other teens in the state to talk about promoting 4-H and developing leadership. Teen Winter Weekend—This February trip travels to Camp Oswagatchie for a weekend of winter activities and offers a chance to meet 4-H’ers from other counties. Equine & Dairy 101—This two day camp allows 4-H’ers to experience college life at SUNY Morrisville with the option to study dairy or equine. Dairy Discovery is a trip to Cornell that offers in-depth information about the dairy industry. 4-H camp is offered in many areas of the state. The 4-H office can assist you with information about camps in an area of your interest. Other 4-H sponsored or approved educational trips as approved by 4-H office.
If you would like to be considered for an award trip scholarship, please fill out page 11. A trip scholarship winner will receive only one award trip scholarship per year. Many trips can still be taken advantage of on a self pay basis. Trips will be awarded each year with consideration to funds available to be used. Please note this awards are non-transferable Trade Show and cannot be shared with another member.
