Mascot Appearance Request Form
Mascot Appearance Request Form Completion of this form is a request only and does not guarantee an appearance. We must receive the request forms at least two weeks prior to the event. There is an appearance fee of $60.00. For events beyond Saskatoon city limits, a mileage fee will be charged. Pre-payment can be accepted via credit card. Event day payment can be made via cash, or cheque (made out to Saskatoon Blades Hockey Club). Payment is due at the completion of the appearance.
