The following compensation program is based upon successful placement of the Biosensor equipment. A "Placement" means the following has been successfully achieved. 1. Personal Presentation to medical office or practitioner and application submitted to Biosensor 2. Approved MedVantage/Biosensor contract has been executed and submitted back to Biosensor 3. To incentivize and reward high levels of personal production MedVantage will allocate $150 per placement nationwide, into the Top 25 Masters Club Fund. At the end of our fiscal year the total funds accumulated in this account will be split equally. 50% will be divided into 8 shares. The highest performer among the Magnificent 7 will receive 2 shares. This will be called the Pinnacle Award. The remaining 6 shares will be allocated to the remaining 6 members of the Magnificent 7. The other 50% will be divided by 18 and shared equally by the remaining 18 members of the Masters Club.
Example: Annual Placements 6000 Allocakon per placement = $150.00 Total Profit Sharing Accounts = $900,000

Tiered Referral Bonus You may choose to refer candidates to MedVantage for consideration as Biosensor Consultants. If you do… Tier 1 -‐ Your personal referrals that have been accepted and contracted with
Tier 1 $250 MedVantage are regarded as your Tier One referrals. Each of their placements will earn you the $250 tier one referral bonus. Tier 2 -‐ Their personal referrals that have been accepted and contracted with
Tier 2 $200 MedVantage are regarded as your Tier Two referrals. Each of their placements will earn you the $200 tier two referral bonus. Tier 3 -‐ And, their personal referrals that have been accepted and contracted with

MedVantage Consultant Residual Income All "placements" that remain in service will generate a monthly residual income that will be paid each month starkng on the 3rd month amer "Equipment Ackvakon". Ackvakon is defined as: ANS1 equipment has been delivered to the physicians office, the MA has been familiarized and the physician is able to perform scans. Ex. If a unit is placed and ackvated on September 15th the residual pay will start on December 15th. If a unit is placed on September 30th the residual will start on December 15th. The residual pay for each unit is $500 as long as the Unit is "In Service". Residual pay will not be earned on equipment in service for a parkal month. The residual commission is paid on a mid-‐month pay cycle. The following page with Consultant Scenarios does NOT include the Residual income. Simply take the annual placements and mulkply the per placement amount of $500 and calculate your residual to add to your earnings.
