About this form: As Caversham LTC is affiliated to the LTA, you can become a British Tennis Member (BTM) automatically and at no extra cost by filling in this form (unless you already have a BTM number or wish to opt out). The information held on the CLTC Database is for the sole use of CLTC and its coaching partner GDT and will not be passed to any other person(s) or organisation outside the Club or GDT without first obtaining your permission. Your name and telephone number may be included in a Membership List to be posted in the Clubhouse or given to other members to enable players to be contacted for arranging games etc (applies to adult members on this form only; details of mini-junior and junior members are not included). Any changes will be displayed on the Notice Board in the Clubhouse. These rules also require members to be bound by the LTA Rules and Disciplinary Code. Proper tennis shoes must be worn on court. Members of Caversham Lawn Tennis Club and their guests use the club facilities at their own risk. Whilst the club makes every effort to ensure that the playing surfaces are safe and without obstruction, members and their guests should check carefully that the courts are free from debris of any kind and that the court surface is safe in the prevailing weather conditions both before commencing play and during play. I agree to supervise and to take full responsibility for the child member(s) (under the age of 11) overleaf while using the courts at CLTC at any time.
