Meridian Park Dizzy Dean JML Baseball Spring 2018 All Star Team Player and Parent Statement of Commitment and Responsibility To be considered for selection to the Dizzy Dean 13U or 14U All Star Team for the spring 2018 season, this form, signed by the parent/guardian, MUST be returned to a Meridian Park Board Member by Thursday May 17, 2018. Please read, initial each item, and sign below. We will submit payment in the amount of $150 for All Star uniform fees during the first week of practice. This is in the form of a check or money order made payable to Meridian Park Baseball, or cash. We will provide an original birth certificate and (2) proofs of residency - a copy of a city or county utility bill, mortgage deed, rental agreement, or phone bill showing proof of residency within 5 calendar days of notification of selection to the All Star Team. We will be responsible for getting our child to practices and games and understand that failure to be available for practices (practices begin soon after teams are announced) and or games can result in the player’s removal from the team. Any player that misses three consecutive practices (practices are several times per week) will be removed from the team. Meridian Park will handle any unusual circumstances such as a death in the family or serious illness on a caseby-case basis. We understand the Dizzy Dean League does not have a “must play” rule in tournament competition. We understand that travel and other expenses will be incurred if the team advances beyond the District tournament. I further understand that I will be financially responsible for all expenses for my child that Meridian Park or Dizzy Dean Baseball does not pay for. We understand that a player’s team becomes “official” when his team plays its first game. At this point, players may NOT participate in any games other than Meridian Park All Star tournament games until the team is eliminated from Dizzy Dean League tournament play. All teams participating in the District tournament for Dizzy Dean are eligible to participate in the State and World Series tournament. The first and second place teams of the State tournament advance to the World Series. We understand that our commitment will be extended to participating in the State tournament as well the World Series should our team advance to that point. In the event that our child is considered for the All Star team, the following conflict(s) could impact his availability for tournament play. (Please indicate tournament dates that would be affected and the conflict. If this area is left blank, the board will assume that your child has no conflicts and can be available for the duration of tournament play.)
